Giriş: Cerrahi stres tepkisi, doku yaralanmasına bilinç dışı bir yanıttır ve travma ya da yaralanma sonrası otonomik, hormonal ve metabolik değişiklikler ortaya çıkar.Amaç: Açık kalp ameliyatı geçirecek hastalara ameliyat öncesi verilen progresif gevşeme eğitiminin cerrahi stres tepkisi/yanıtı üzerindeki etkisini belirlemek amacı ile yapıldı.Yöntem: Deneysel olarak yapılan çalışmaya 39 deney, 39 kontrol olmak üzere toplam 78 hasta katıldı. Çalışmada veriler kişisel bilgi formu, Spielberger'in durumluluk anksiyete ölçeği ve stresin fizyolojik bulgularını değerlendirme formu kullanılarak toplandı. Deney grubuna ameliyat öncesi üç gün öğleden önce ve sonra olmak üzere günde iki kez 30 dakika progresif gevşeme eğitimi uygulandı. Kontrol grubuna ise herhangi bir uygulama yapılmadı. Veriler SPSS 11.5 MEDCALC paket programında chi-square, korelasyon, paired simple t testleri kullanılarak değerlendirildi. Anlamlılık düzeyi p<0.05 kabul edildi.Bulgular: Çalışma kapsamında yer alan deney grubunun yaş ortalaması 63.64±8.78, kontrol grubunun yaş ortalaması 64.89±7.67 idi. Çalışmada deney grubundaki hastaların %58.9'unun, erkek, %38.4'ünün ilköğretim mezunu, %69.2'si daha önce hastanede yattığı ve %43.5'inin ameliyat deneyimi olduğu belirlendi. Kontrol grubunun %64.1'inin erkek, %53.8'inin ilköğretim mezunu olduğu, %64.1'inin daha önce hastanede yattığı, %58.9'unun, ameliyat deneyimi olduğu belirlendi. Çalışmada preoperatif dönemde deney grubuna uygulanan gevşeme egzersizinin, her iki grup karşılaştırıldığında yaşam bulgularına anlamlı etkisi olmadığı (p>0.05) ancak, deney grubunda kontrole göre postoperatif dördüncü gün kan glukoz değerinin, postoperatif birinci ve dördüncü gün kortizol ve SAI değerlerinin düşük olduğu ve gruplar arasındaki farkın istatistiksel olarak anlamlı olduğu bulundu (p<0.05).Sonuç: Bu çalışma grubunda yer alan hastalara preoperatif dönemde bir kalp-damar cerrahi hemşiresi tarafından yapılan progresif gevşeme egzersizinin hastaların cerrahi stres tepkisi değerleri üzerinde olumlu yönde etkili olduğu bulundu. Bu bağlamda açık kalp ameliyatı geçirecek hastaların bakım kalitesinin arttırılması için progresif gevşeme egzersizi yapılmasının yararlı olduğu söylenebilir.Anahtar Kelimeler: Progresif gevşeme, cerrahi stres tepkisi/yanıtı, açık kalp ameliyatı
Introduction:Surgical stress response is a unconscious response to the tissue injury and autonomic, hormonal and metabolic alterations occur after trauma or injury.Objective:This study was designed for determining the effect of progressive relaxation training given prior to surgery on surgical stres response to the patients who underwent open heart surgery.Methods:A total of 78 patients attended to this experimental study in which 39 were the study group and the rest were the control group. The datas in the study were gathered using personal information forms, Spielberger?s state-anxiety scale and physiological findings of stress assessment forms. Progressive relaxation training were given to the study group three days prior to the surgery, scheduled as 30 minutes twice a day, in the morning and afternoon. No procedures were done to the control group. The datas were assessed with chi-square, correlation and paired simple t-test using SPSS 11.5 MEDCALC programme. The level of significance were accepted as p<0.05.Results: Mean age of the study group was 63.64±8.78 years while it was 64.89±7.67 years in the control group. It was determined that for the patients in the study group, 58.9% were male and 38.4% were primary school graduates, whereas in the control group, these percentages were 64.1% and 53.8%, respectively. In addition to this, of these patients in the study group, 69.2 % had prior history of hospitalization and 43.5% underwent a prior surgical procedure. However, it was detected that 64.1% of the patients in the control group reported prior hospital admittance and 58.9% underwent any kind of surgery. In this study, it was found that relaxation trainings in the study group perfomed during preoperative period made no significant difference in vital signs (p>0.05) between both groups, however, in the study group, blood glucose levels in the postoperative fourth day and cortisol and SAI levels in the postoperative first and fourth day were lower than in the control group. These differences between the two groups were statistically significant (p<0.05).Conclusion: Progressive relaxation training performed to the patients in the study group by a cardiovascular surgery nurse improved the surgical stress response of the patients. In this context, it can be suggested that progressive relaxation trainings will be beneficial to improve the care quality of the patients who will undergo open heart surgery,Key Words: Progressive relaxation, surgical stress reaction / response, open-heart surgery
