Amaç: Bu çalışmada amacımız, İAK-MRG incelemesi gerçekleştirilen
hastalarda insidental bulguları ve sıklığını saptamaktır.
Gereç ve Yöntem: 02.11.2015 ile 30.10.2017 tarihleri arasında, kulak burun
boğaz ve nöroloji polikliniklerinden, akustik nörinom şüphesi, işitme kaybı, baş
dönmesi, kulak çınlaması, Bell paralizisi, PSK patolojisi ön tanıları, klinik bilgileri
ve şikayetleri ile İAK-MRG için Radyoloji Anabilim Dalı'na yönlendirilen ve
MRG'leri elde edilen 387 hastanın görüntüleri retrospektif olarak
değerlendirilildi. Yetersiz görüntü kalitesi, dışlama kriteri olarak kabul edildi.
Çalışma grubu için yaş ve cinsiyet sınırlaması yoktu. Radyografik incelemeler
Picture Archiving and Communication Systems (PACS) üzerinden retrospektif
olarak yapıldı. Hastaların yaş, cinsiyet bilgileri, başvuru anındaki şikayetleri, ön
tanıları, klinik bilgileri hasta dosyalarından dökümente edildi. Taranan MRG'deki
insidental bulgular ICD-10 tanı kodlarına göre tanımlandı.
Bulgular: İAK-MRG çekilen toplam 387 hastanın yaş ortalaması 54,87±16,99
yıl olup hastaların 207'si (%53,5) erkek, 180'i (%46,5) kadındı. Çalışmaya dahil
edilen hastaların 306'sında (%79,1) bir veya birden fazla insidental bulgu
saptandı. Bu insidental bulgular sıklık sırasıyla; beyaz cevher hiperintensiteleri
(%38,56) (hiperintens nonspesifik milimetrik odaklar %19,93, muhtemel kronik
iskemik sinyal değişiklikleri %18,63), sinüzit (%29,08), serebral atrofi bulguları
(%25,82), mastoidit (%24,18), vasküler loop (%18,63), parsiyel empty sella
(%4,58), vertebral arter hipoplazisi (%4,90), otitis media (%3,27), anevrizma
(%2,61), araknoid kist (%1,96), anterior serebral arter hipoplazisi (%1,63),
hidrosefali (%1,63), intrakraniyal kitle (%3,59), empty sella (%0,98), internal
karotid arter oklüzyonu (%0,65), glomus jugulare (%0,65), dural sinüs içi
trombüs (%0,33), gelişimsel venöz anomali (%0,33), mondini malformasyonu
(%0,33), vertebral arter oklüzyonu (%0,33), sağ oksipital lobda ensefalomalazi
(%0,33), beyin sapı basısına neden olan baziler arter anevrizması (%0,33),
baziler arter stenozu (%0,33), fetal tip posterior serebral arter (%0,33), kavernöz

sinüste asimetrik kontrastlanma (%0,33), serebellar venöz anomali (%0,33),
hemorajik kontüzyon (%0,33) ve diğer insidental bulgular (nazofarinks posterior
duvarda kalınlaşma, ince lensler, optik sinir perinörinal BOS mesafesinde artış,
internal karotid arterin sfenoid sinüse protrüzyonu, mega sisterna magna, optik
nörit, sol parotis bezi kitle) şeklindeydi.
Sonuç: Çalışmamızda, diğer çalışmalara oranla insidental bulgular daha
yüksek oranlarda bulunmuştur ve bunun nedeni insidental bulguları daha geniş
ve ayrıntılı listelemiş olmamız ve semptomatik hasta grubu ile çalışmış olmamız
olabilir.

Purpose: The purpose of this study was to determine incidental findings and
their frequencies on internal acoustic canal MRI scan.
Material and methods: After the approval of the ethics committee of the clinical
studies in the Faculty of Medicine of Çanakkale Onsekiz Mart University have
been performed , the images of 387 patients who were referred to Radiology
Department for IAC MRI scan from ear nose throat and neurology clinics with
diagnosis, clinical information, and complaints of clinical suspicion of acoustic
neurinoma, hearing loss, dizziness, tinnitus, Bell's paralysis and suspected
patologies of PSC were retrospectively evaluated. Insufficient image quality was
considered as exclusion criteria. There was no age and gender limitation for the
study group. Radiographic examinations were performed retrospectively on
Picture Archiving and Communication Systems (PACS) in Radiology
Department. Patients' age, gender information, complaints at the time of
admission, preliminary diagnoses, clinical information were documented from
patient files. Incidental findings on scanned MRI were defined according to ICD-
10 diagnostic codes.
Results: In a retrospective study of 387 patients who underwent MRI, the mean
age of patients was 54,87 ± 16,99 and 207 (%53,5) of the patients were male
and 180 (%46,5) were female. One or more incidental findings were found in
306 (%79,1) of the patients included in the study. These incidental findings (n =
306) were determined as respectively; white matter hyperintensities (%38,56)
(hyperintense nonspecific millimetric foci 19,93%, possible chronic ischemic
signal changes %18,63), sinusitis (%29,08), cerebral atrophy findings (%25,82),
mastoiditis (%24,18), vascular loop (18,63%), partial empty sella (%4,58),
vertebral artery hypoplasia (%4,90), otitis media (%3,27), aneurysm (%2,61),
arachnoid cyst (%1,96), anterior cerebral artery hypoplasia (%1,63),
hydrocephalus (%1,63), intracranial mass (%3,59), empty sella (%0,98), internal
carotid artery occlusion (%0,65), glomus jugulare (%0,65), thrombosis of dural
sinuses (%0,33), developmental venous anomaly (%0,33), mondini
malformation (%0,33), vertebral artery occlusion, encephalomalasia of right
occipital lobe (%0,33), basilar artery aneurysm causing brainstem compression
(%0,33), basilar artery stenosis (%0,33), fetal type posterior cerebral artery
(%0,33), asymmetric enhancement of the cavernous sinus (%0,33), cerebellar
venous anomaly (%0,33), hemorrhagic contusion (%0,33), and other incidental
findings (posterior wall thickening of nasopharynx, thin ocular lenses, increase
in the CSF distance of the optic nerve, sphenoid sinus protrusion of the internal
carotid artery, mega cisterna magna, optic neuritis, left parotid mass).
Conclusion: In our study, incidental findings were found to be higher than other
studies, and this may be because we have listed the incidental findings in
greater detail and have worked with a symptomatic group of patients.
