Amaçlar Alt ekstremite cerrahileri için genel anestezi ve santral blok uygulamaları halen en yaygın kullanılan yöntemlerdir. Bununla birlikte ultrasonografi eşliğinde kombine femoral siyatik blok uygulamaları güncel bir alternatif olarak popülerliğini sürdürmektedir. Bu çalışmanın amacı kliniğimizde de uygulanmakta olan bu yöntemin sonuçlarını araştırmaktır. Hastalar ve Yöntem İ.Ü.Tıp Fakültesi Anesteziyoloji Kliniğinde ASA skoru I-IV arası 143 hasta çalışmaya dâhil edildi. Çocuk, kanama hastalığı ve lokal anestezik allerjisi olan hastalar çalışma dışı bırakıldı. Ultrason ve nörostimülatör kullanılarak kombine siyatik femoral blok uygulandı. Klasik yöntemle uygulanan her iki bloktada 20-40 ml %0,25 bupivakain + %1 lidokain/prilokain tercih edildi. Blok uygulama, hastanın cerrahiye teslim ve cerrahi işlem süreleri kaydedildi. Hemodinamik komplikasyonlar, uygulama ile ilgili komplikasyonlar ve başarı oranları irdelendi. Sonuçlar %95 güven aralığında, anlamlılık p<0,05 düzeyinde değerlendirildi. Bulgular Hastaların yaş ortalaması 54,46 ± 17,13 yıl idi. İşlem süresi ortalama 13,9 ± 3,64 dk ve operasyon süresi ise ortalama 66,62 ± 25,36 dk olarak gerçekleşti. Hastanın cerrahiye teslim süresi ortalama 35,3 ± 3,22 dk oldu. Hastaların kan basıncı, SpO2 ve nabız ortalamalarında istatistiksel olarak anlamlı değişiklikler gözlenmedi. VAS ölçümleri takiplerine göre istatistiksel olarak anlamlı farklılık görüldü (p<0,01). Olgularımızın %95,8 başarılı (tam + kısmi başarılı) olarak değerlendirildi. Beş olguda nöral enjeksiyon düşünüldü. Sonuçlar Femoral siyatik kombine blok uygulaması mevcut anestezi yöntemlerine alternatif, yüksek başarı oranlarına sahip güncel bir yaklaşımdır. Bu çalışmada başarı oranımız (tam + kısmi başarı) %95,8 olup erken dönemde majör bir komplikasyon görülmemiştir. Bu durum blokların ultrasonografi kullanılarak yapılmasına bağlıdır. Bu yöntem yaygınlaşırken nörostimülasyonunda tamamlayıcı bir yöntem olarak kullanılması hasta güvenliğini arttırır.
ABSTRACT Objective General anesthesia and central block applications for lower extremity surgery are still the most commonly used methods. However, ultrasound-guided combined femoral sciatic block continues its popularity as an alternative to current practices. The purpose of this study was evaluate to our results of this method. Patients and Methods 143 patients with an ASA score of I-IV were included in this study at our Anestesiology department. Children, patients with allergic to the local anesthetics used or bleeding disorders were excluded from the study. Ultrasound combined sciatic and femoral block was performed with the aid of the neurostimulator. Combined sciatic-femoral block was obtained with 20-40 ml of 0.25% bupivacaine + 1% lidocaine / prilocaine. Block applications, delivery time of to the patients' to surgaery and surgical procedure time were recorded. Hemodynamic complications associated with the application and success rates of were evaluated. Results at 95% confidence interval, significance of p <0.05 were evaluated. Results The mean age of patients was 54,46 ± 17,13 years, block performence time was 13,9 ± 3,64 min and the mean duration of the operation was 6,62 ± 25,36. The patient's mean delivery time to surgery was 35,3 ± 3,22 min. The patients' blood pressure, SpO2 and pulse rate values were found without any statistically significant change in the average. VAS measurements showed statistically significant differences during the follow-up periods (p <0,01). 95.8% of our cases were evaluated as successfull (complete success + partial success). Five cases were considered as neural injections. Conclusion Combined sciatic femoral block anesthesia is a good alternative to current practices, with a high success rate: the success rate in the present study is 95,8% without any significant major complications. This result is due to use of ultrasound during block application. As this technique becomes widespread the use neurostimulator as complementary device increases patients' safety.
