ÖZET Bu çalışmada Bell felci prognozunu belirlemede daha önce kullanılmamış bir elektrofizyolojik test olan motor ünite sayı tahmini yöntemi (MUNE), oldukça kabul görmüş bir diğer elektrofizyolojik testle (uyandırılmış elektromiyografi=EEMG) karşılaştırılmaktadır. Bell paralizisi ile başvuran 53 kadın, 47 erkek (yaş: 36.3±16.5) hasta ile 13 erkek, 10 kadm (yaş: 36.6±1 1.3) kontrol olgusu incelendi. Olguların sağ ve sol m.nasalislerinde fasial sinir CMAP amplitüdleri ve modifiye McComas yöntemi ile MUNE değerleri belirlenip iki tarafa ait değerler oranlandı. Çalışmanın ilk aşamasında normal kontrollerde iki farklı zamanda bilateral m. nasalisten MUNE kayıtlamaları yapılarak bu yöntemin yüz sinirinden innervasyon alan bir kasta simetrik ve tekrarlanabilir olduğu gösterildi. Çalışmanın ikinci aşamasında Bell felci ve Ramsay Hunt sendromundan oluşan akut periferik yüz felci olgularında EEMG verileri ve MUNE değerleri karşılaştırıldı. İncelemeler hasta grubunda yüz felci yerleştikten sonra 6, 8, 11, 14, 20, 30. günlerde ve sonra ayda bir tekrarlandı. Hastalar, semptomatik tarafta CMAP amplitüd kaybı %70'in altında (Grup I, n=67) ve üstünde (Grup II, n=33) olanlar şeklinde ikiye ayrıldı. İlk 6 aylık takipte Grup I ve H'de semptomatik ve asemptomatik taraflarda CMAP amplitüdü ve MUNE kaybı oranlarının paralel seyrettiği, ancak erken dönemde CMAP kaybının MUNE kaybma göre anlamlı derecede daha yüksek olduğu gözlendi (11. günde semptomatik / asemptomatik taraf CMAP-MUNE oranlan I. Grupta: 0.57-0.64, II. Grupta 0.28-0.42). Zaman içinde klinik düzelme ile birlikte semptomatik ve asemptomatik tafalarm CMAP amplitüdleri -Grup I'de daha erken olmak üzere-her iki grupta hemen hemen eşit hale gelip, oranlan "l"e yaklaşırken MUNE oram daha düşük değerlerini sürdürmekteydi. CMAP ve MUNE oranlan arasındaki farklılık nedeniyle, EEMG ve MUNE yöntemlerinin sinir hasan ve iyileşmesinin kısmen de olsa değişik özelliklerini yansıttığı düşünüldü. Akut periferik yüz felçlerinin geç dönemlerinde fonksiyonel iyileşmeyi EEMG yönteminin, hasarlı aksonların rejenerasyonunu ise MUNE'nin daha iyi yansıttığı kanısına vanldı. ABSTRACT In this study, an electrophysiological test (motor unit number estimation=MUNE) that has not been used previously to estimate the prognosis of Bell's palsy was compared with another well established electrophysiological test (evoked electromyography EEMG). Fifty three female, 47 male (mean age: 36.3±16.5 years) patients with Bell palsy or Ramsay Hunt syndrome, and 10 female, 13 male (mean age: 36.6±1 1.3 years) healthy control subjects were evaluated. Facial nerve CMAP amplitude and MUNE (according to modified McComas method) values were obtained from right and left nasalis muscles. The ratios of the CMAP or MUNE values elicited on the symptomatic sides to those of the asymtomatic sides were calculated. In the first phase of the study, MUNEs of the control subjects were recorded twice on different occasions and it was shown that the electrophysiological method has symmetrical and reproducible results in a muscle innervated by facial nerve. In the second phase, EEMG and MUNE data of the cases with acute idiopathic peripheral facial palsy (Bell'palsy and Ramsay Hunt syndrome) were compared. Electrophysiological examinations were performed on the 6th, 8th, 1 1th, 14th, 20th, 30th days of the facial paralysis and repeated once a month. The patients were divided into 2 groups: Group I (n=67) with < 70% loss of CMAP amplitude and Group II (n=33) with >70% loss of CMAP amplitude. In 6 months period, CMAP amplitude and MUNE ratios of symptomatic and asymptomatic sides were parallel to each other in Group I and Group II. However, in the early period of facial palsy loss of CMAP amplitude was significantly higher than MUNE loss (on the 1 1th day, symptomatic / asymptomatic side CMAP- MUNE ratios were: Group I: 0.57-0.64, Group D: 0.28-0.42). With the clinical recovery, the ratio of CMAP amplitude was increased to "1" in both groups but the ratio of MUNE maintained in lower values. This difference between the results of two methods implied that they reflect different characteristics of nerve injury and recovery. It may be thought that EEMG more reliably indicates functional recovery and MUNE reflects the regeneration of injured axons in late periods of acute peripheral facial palsy.
