Amaç: Ekstremite cerrahisi sırasında genel ve spinal anestezi uygulanan
hastalarda postoperatif kognitif fonksiyon etkilerinin gruplar arasında
değerlendirilmesidir.
Materyal ve metod: Çalışma 18-65 yaş arası, ASA 1-2 sınıfı, elektif cerrahi
geçiren 60 hasta ile yapıldı. Hastalar, genel anestezi uygulanan (n=30) ve spinal
anestezi uygulanan (n=30) olmak üzere iki gruba ayrılmıştır.
Hastaların ameliyat öncesi ve postoperatif dönemde 4. ve 24. saatte
Standardize Mini Mental Test (MMT) ve Montreal Bilişsel Değerlendirme Ölçeği
(MoCA) ile kognitif fonksiyonları değerlendirilmiştir. Ayrıca, hastaların anestezi
değerlendirme formlarından boy, kilo, yaş ve ASA skorları, kullandığı ilaçlar ve
komorbidite gibi değişkenleri ile anestezi takip formlarından anestezi sırasında
kullanılan ilaçlar, nabız, kan basıncı ve oksijen satürasyonu gibi değişkenler
kaydedilmiştir.
Genel anestezi indüksiyonu ve spinal anestezi yapılmadan önce kalp tepe
atımı, sistolik arter basıncı, diastolik arter basıncı, ortalama arter basıncı ve SPO2
kaydedilerek operasyon sonuna kadar 10 dakikada bir kayıt tutulmuştur.
Bulgular: Hastaların demografik özelliklerinin gruplara göre dağılımı
incelendiğinde; cinsiyet, yaş grubu, ASA, eğitim durumu, yaş, eğitim süresi ve
cerrahi süre ile gruplar arasında herhangi bir anlamlı ilişki bulunmamıştır. MoCA ve
MMT değerlerini gruplar arasında karşılaştırdığımızda anlamlı bir fark olmadığı
bulunmuştur (p> 0,050). Grup içerisinde MoCA ve MMT değerlerini
karşılaştırdığımızda ise grup genelde başlangıç değerlerine göre 4. saatteki
değerlerinde azalma olduğu gözlemlenmiştir (p<0,01)
Sonuç: Ekstremite cerrahilerinde uygulanan genel anestezi postoperatif erken
dönemde kognitif fonksiyonlarda azalmaya neden olabilir.

Objectives: The aim of this study was to evaluate postoperative cognitive functioning among patients who underwent general and spinal anesthesia during the extremity surgery.
Material and Method: The study was conducted with 60 patients aged 18-65 years, ASA 1-2 class, undergoing elective surgery. Patients were divided into two groups, general anesthesia (n = 30) and spinal anesthesia (n = 30).
Cognitive functions were assessed by Standardized Minimental Test (MMT) and Montreal Cognitive Assessment (MoCA) tests at 4th and 24th hours in preoperative and postoperative periods. In addition, variables such as height, weight, age and ASA scores, medications used and comorbidities and anesthesia follow-up forms were recorded from the anesthesia evaluation forms of the patients and variables such as pulse, blood pressure and oxygen saturation.
Before general anesthesia induction and spinal anesthesia, heartbeat, cystolic arterial pressure, diastolic arterial pressure, mean arterial pressure and SPO2 were recorded every 10 minutes until the end of the operation.
Results: We did not find any significantly correlation in demographic datas, ASA, education status, ASA and surgery time between groups. Also there was no significant difference between MoCA and MMT values between the groups. When we compared the MoCA and MMT values with in the group, it was observed that in the general anesthesia group, the values at 4th hour decreased compared to the baseline values.
Conclusion: In conclision general anesthesia, may cause decrease in cognitive functions in early postoperative period after extremity surgery.
