Acil Birimlerde Hastaların Triyaj ve Hemşirelik Uygulamalarından

Araştırma, acil birimlerde hastaların triyaj ve hemşirelik uygulamalarından memnun
olma durumlarını değerlendirmek amacıyla planlandı ve gerçekleştirildi.
Tanımlayıcı ve ilişki arayıcı olarak tasarlanan araştırmanın örneklemini, 15-31 Aralık
2013 tarihleri arasında Kocaeli'de bir devlet hastanesinin acil servisine başvuruda bulunan ve
çalışmaya katılmayı kabul eden 115'i kadın, 83'ü erkek olmak üzere toplam 198 hasta
oluşturdu. Araştırma kriterlerine uyan 198 hastaya araştırmanın amacı, içeriği ve yöntemi
açıklanarak, Triyaj Memnuniyet Skalası bulunan bireysel özellikler formu ile Newcastle
Hemşirelik Bakımı Memnuniyet Ölçeği'nden (NHMÖ) oluşan veri toplama formu verildi.
Verilerin değerlendirilmesinde, tanımlayıcı istatistiksel yöntemlerin yanı sıra parametrik ve
parametrik olmayan testler kullanıldı.
Yapılan değerlendirmeler sonucunda, hastaların acile geliş nedenleri arasında, basit
nedenler (küçük sıyrıklar, hafif bulantı vb.), şiddetli karın ağrısı, kusma ve ishal şikayetlerinin
ilk üç sırayı aldığı; çoğunluğunun triyaj uygulaması için beklemediği ve triyaj kategorisine
uygun sürede tedavi aldıklarını belirttikleri; hemşirelik bakımı ile ilgili NHBMÖ
ortalamasının 73,34±17,66 (min-mak= 19-100 puan), Triyaj Memnuniyet Skala
ortalamasının ise 7,37±2,11 (min – mak = 1-10 puan) olduğu, hastaların yapılan
uygulamalardan memnun olduğu belirlendi. Memnuniyet düzeylerinde, sosyal güvencesinin
olması, başvuru saati (08:00-16:00), bekleme süresi, doğru triyaj kategorisine yönlendirme,
uygun sürede tedavi ve bakım hizmeti verilmesi, sağlık sorununu algılama (çok ciddi/ ciddi/
ciddi değil) gibi faktörlerin etkili yanı sıra sağlık durumuna ilişkin bilgilendirme ve psikolojik
destek boyutlarında memnuniyet düzeylerinin düşük olduğu bulundu.
Bu sonuçlar doğrultusunda, hemşirelik uygulamalarına ilişkin memnuniyet
analizlerinin, dinamik bir yapıya sahip, ani gelişen ve yaşam için risk oluşturabilen sağlık
sorunu olan bireylerin hizmet aldığı acil birimlerde, periyodik olarak uygulanması, gerekli
düzenlemelerin yapılması ve eksikliklerin giderilmesi, önerilebilir.

Satisfaction of Patients with Triage and Nursing Practice In The

The purpose of this study was to identify the satisfaction of patients with triage and
nursing practice in emergency departments.
The sample of this descriptive study was made up of 198 patients (115 women and 83
men) that visited an emergency department at a public hospital in Kocaeli between December
15 and 31 December, 2013 and agreed to participate in the study. The participants had been
informed about the purpose, content and methods of this study. Then, the Data Collection
Form was administered to the patients. The form consisted of two subsections, namely
Personal Characteristics Form that included Visual Analogue Satisfaction Scale (VASS) and
Newcastle Satisfaction with Nursing Scale (NSNS). Data analysis involved descriptive
statistical methods as well as parametric and nonparametric tests.
The results showed that simple complaints (small scrape, nausea etc.), acute
stomachache, vomiting and diarrhea were the first three reasons for visiting the emergency
department. The majority of the patients reported not waiting for triage and getting treatment
in a time period appropriate for triage category. The NSNS total mean score was 73,34±17,66
(min-max=19-100) and the VASS total mean score was 7,37±2,11 (min-max=1-10). The
patients reported their overall satisfaction with the practices. The satisfaction levels of the
patients were influenced by factors such as health insurance, application time (8am- 4pm),
waiting time, directing to right triage category, treatment and nursing in a reasonable time and
identification of health problems (so serious/serious/not serious). There were low levels of
patient satisfaction for psychological support and information about their health status.
In light of the findings, satisfaction with nursing practice should be regularly analyzed
in emergency departments, which are dynamic and serve patients who are at risk of high
mortality. The results of such analyses should be used to revise the emergency nursing
practice at these departments.
