Kolorektal kanserlerin erken dönemde belirlenmesi için tarama programları bilinmesine ve önerilmesine rağmen erişkin bireylerde dışkıda gizli kan testi ve kolonoskopi yaptırma oranları oldukça düşüktür. Bu araştırmanın amacı 50-70 yaş arasındaki bireylere uygulanan Hemşire Navigasyon Programı'nın kolorektal kanser tarama davranışlarına (dışkıda gizli kan testi ve kolonoskopi yaptırma) ve kolorektal kanser tarama davranışları ile ilgili sağlık inançlarına etkisini incelemekti. Ön test-son test kontrol gruplu, tekrarlayan ölçümlü deneysel çalışmanın örneklemini İstanbul'da bir Aile Sağlığı Merkezinden hizmet alan 50-70 yaş arası 110 birey oluşturdu. Deney grubuna Transteoretik Model temel alınarak geliştirilmiş Hemşire Navigasyon Programı uygulandıktan sonra 3 ve 6.ayda deney ve kontrol grubu sonuç değişkenleri yönünden karşılaştırıldı. Veriler 'Tanıtıcı Özellikler Bilgi Formu, Harward Kolorektal Kanser Riski Değerlendirme Aracı, Kolorektal Kanser Tarama Davranışları Yarar ve Engel Algısı Ölçeği' kullanılarak toplandı. Verilerin analizi bilgisayarda tanımlayıcı ve önemlilik analizleri kullanılarak yapıldı. Yaş ortalaması 59,18±5,25 olan bireylerin %65,5'i kadın, evli (%82,7), yüksekokul mezunu ve daha üst düzey eğitime sahipti (%36,4). Hemşire Navigasyon Programı uygulanan grupta 3 ve 6.ay izlemlerde DGKT yaptırma (%82 ve %84) ve kolonoskopi yaptırma oranı (%15 ve %22) kontrol grubuna gore anlamlı olarak daha yüksek bulundu. Kontrol grubu ile karşılaştırıldığında deney grubunun engel algısı puan ortalaması anlamlı olarak daha düşük, yarar algısı puan ortalaması anlamlı olarak daha yüksek bulundu. Sonuç olarak bu çalışma, Hemşire Navigasyon Programı'nın kolorektal kanser tarama davranışlarında ve KRK tarama davranışı ile ilgili sağlık inançlarında önemli etkisi olduğunu göstermektedir. Hemşire Navigasyon Programı'nın birinci basamak sağlık hizmetlerinde farklı gruplarda uygulanarak etkinliğinin değerlendirilmesi önerilir.
Although screening programs are known and recommended for the early detection of colorectal cancers, screening rates of fecal occult blood tests and colonoscopy are very low in adult individuals. The aim of this study is to evaluate effects of Nurse Navigation Program that is administered to individuals aged 50-70 on their screening behaviour concerning colorectal cancer (fecal occult blood test and colonoscopy) and their beliefs about this process. The study was designed as a pre-test and post-test with control and experiment group. The participants of the study were 110 individuals aged between 50 and 70 registered to a family health center in İstanbul. The 'Nurse Navigation Program' was administered to the participants. The data were collected through the use of the following three tools: the Demographical Information Form, The Harward Colorectal Cancer Risk Assesment Tool, Instruments to Measure Colorectal Cancer Screening Benefits and Barriers. The data obtained were analysed by the descriptive and significance analyses. Following the Nurse Navigation Program the experimental groups FOBT (82% and 84%) and colonoscopy completion rate (15% and 22%) were significantly higher in the control group at 3 and 6 months follow-up. Following the program when compared to the control group it is found that the mean score of the barriers perception of the experimental group was significantly lower and the mean score of the benefits perception of the experimental group was significantly higher. The results show that the Nurse Navigation Program had significant effects on the CRC screening behaviour and health-related beliefs concerning the CRC screening. It can be suggested that the Nurse Navigation Program can be further tested on different groups to observe its effects.
