Amaç: Adolesan döneminde otoimmün tiroiditin patogenezinde Parvovirus B19'un tetikleyici rolü araştırılması amaçlandı. Gereç ve Yöntem: Ağustos 2015-Ağustos 2016 tarihleri arasında, İstanbul Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı, Genel Pediatri Bilim Dalı'na ve Büyüme Gelişme ve Pediatrik Endokrinoloji Bilim Dalı'nda yapıldı. Çalışmaya 10-18 yaş arası, son 6 ay içinde Hashimoto tiroiditi tanısı alan ve ailelerinden yazılı onam alınan 35 hasta dahil edildi. Parvovirus B19 enfeksiyonu ile ilişkilendirilebilecek akut hastalığı olmayan sağlıklı, fizik muayenede guatr saptanmamış, ailede tiroid hastalığı öyküsü olmayan ve tiroid fonksiyon testleri normal olarak saptanmış, rastgele yöntemle seçilen 35 çocuk kontrol grubu olarak çalışmaya alındı. Hastalık süresi ve seyri ile ilgili bilgi, ek hastalıklar, ilaç kullanımı, döküntülü hastalık, ailede diğer otoimmün hastalıklar sorgulandı, bilgiler kaydedildi. Gönüllülerden Parvovirus IgG, Parvovirus IgM ve Parvovirus PZR için kan örneği alındı. İstatistiksel analizler için SPSS (Statistical Package for Social Sciences) paket programı 21.0 versiyonu kullanıldı. Anlamlılık p<0,05 düzeyinde değerlendirildi. Bulgular: Hasta grubunun %80'i (n: 28) kız, %20'si (n: 7) erkek ve kontrol grubunun %68,6'sı (n: 24) kız, %31,4'ü (n: 11) erkek idi. Çalışmaya dahil edilen hastaların ortalama yaşı 158,63±26,6 ay saptandı. Tanı anındaki yaş ortalaması 155,8±25,96 ay idi. Tanı anı ve serum örneklerinin toplanması arasında geçen süre ortalama 2,8 ± 1,8 ay saptandı. Kontrol grubunun ortalama yaşı ise 161,9±26,3 ay saptandı. Tiroid işlevlerine göre, tanı anında Hashimoto tiroiditli olguların 10'u (%28,6) ötiroidik, 15'i (%42,9) subklinik hipotiriodi, 6'sı (%17,1) hipotiroidi ve 3'ü (%8,6) hipertiroidi idi.1 olguda tanı anında bakılan TSH ve sT4 düzeyi normalin üst sınırında idi. Hashimoto tiroiditi olan hastaların %25,7'sinde (n:9) anti-TPO antikoru, %17,1'inde (n:6) anti-TG antikoru ve %37,1'inde (n:13) her iki antikorda pozitif saptandı. Ultrasonografik değerlendirmede olguların % 14,3'ünde (n:5) tiroid bezi volümünde artış vardı. Tiroid parankiminde heterojenite tüm olgularda gözlendi. Pseudonodüler görünüm 16 olguda (%45,7) saptandı. Parvovirüs IgM antikoru 4 (%11,4) hastada ve 5 (%14,3) kontrol olgusunda, Parvovirüs IgG antikoru 13 (%37,1) hastada ve 6 (%17,1) kontrol olgusunda pozitif saptandı. Hashimoto tiroiditli hastalarda PV-B19 DNA %11,4 (4/35), kontrol grubunda %14,3 (5/35) pozitif olarak saptandı. Antikor ve PZR pozitifliği açısından kontrol ve hasta grupları arasında anlamlı bir fark yoktu. Çoklu doğrusal regresyon analizinde Parvovirus B19 IgG düzeyi üzerine sT4 ve anti-TPO antikor düzeylerinin etkili olduğu görüldü. Sonuç: Hashimoto tiroiditi genetik, endojen ve çevresel faktörlerin etkileşimi sonucunda geliştiği düşünülmektedir. Parvovirus-B19'un teorik olarak otoimmün tiroid hastalığını tetikleyebileceği düşünülmesine rağmen Hashimoto tiroiditi patofizyolojisinde Parvovirus-B19'un rolüne dair yeterli kanıt yoktur. Hashimoto tiroiditi ile Parvovirus B19 arasındaki ilişkiyi değerledirdiğimiz bu çalışmada; Parvovirus B19 ELISA ve PZR sonuçlarına göre hasta ve kontrol grupları arasında istatistiksel açıdan anlamlı fark yoktu ancak HT olgularında %37,1 oranında Parvovirus IgG pozitif olması geçirilmiş Parvovirus B19 enfeksiyonunun HT oluşumunu tetikleyen faktörlerden biri olabileceğini düşündürtmektedir. Bu ilişkinin gösterilmesi amacıyla daha fazla sayıda hasta ile yapılan prospektif kohort çalışmaların faydalı olacağını düşünmekteyiz. Objectives: The purpose of this study was to determine the trigger role of parvovirus B19 in the pathogenesis of autoimmune thyroiditis in adolescent period. Materials and Methods: The study was run in the Department of General Pediatrics and the Department of Pediatric Endocrinology, İstanbul Medical Faculty between August 2015 and August 2016. 35 patients who are 10-18 years old with a diagnosis of Hashimoto thyroiditis in the last 6 months were included in the study. 35 healthy volunteers who have no acute disease associated with Parvovirus B19, no goiter on physical examination, no family history of thyroid disease and have normal thyroid function tests were included as control group. Written consent was obtained from all subjects. Each participant was questioned disease duration, drug use, medical history, history of childhood eruptive diseases, family history of autoimmune diseases. Serum samples drawn from participants and were tested for IgM and IgG antibodies against Parvovirus B19 and Pavovirus B19 DNA. Statistical analysis was performed using SPSS (Statistical Package for Social Sciences) package programme 21.0. We considered p<0,05 to be statistically significant. Results: %80 (n: 28) of patients with HT was female and %20 (n: 7) was male. %68,6 (n: 24) of control group was female and %31,4 (n: 11) was male. The mean patient age was 158,6±26,6 months. The mean age at the time of diagnosis was 155,8±25,9 months. The mean time from the diagnosis of HT was 2,8 ± 1,8 months. The mean age of control group was 161,9±26,3 months. In terms of thyroid function, 10 (%28,6) patients were euthyroidism, 15 (%42,9) had subclinical hypothyroidism, 6 (%17,1) were obvious hypothyroidism and 3 (%8,6) hyperthyroidism. Anti-TPO antibodies was positive in %25,7 (n:9) of patients, anti-TG antibodies were positive in %17,1 (n:6), whereas both antibodies were detected in %37,1 (n:13). In ultrasonography; % 14,3 of patients had thyroid enlargement. Thyroid parenchymal heterogeneity was seen in all patients. Pseudonodular appearance was seen in 16 (%45,7) patients. Parvovirus IgM antibodies were positive in 4 (%11,4) patients and 5 (%14,3) controls while Parvovirus IgG antibodies were present in 13 (%37,1) patients and 6 (%17,1) controls. Parvovirus B19 DNA was detectable in %11,4 of patients (n:4) and %14,3 controls (n:5). There was no significant difference between the patient and control groups in terms of antibody and PCR positivity. Using multiple linear regression analysis, concentrations of Parvovirus B19 IgG were associated with fT4 and anti-TPO antibodies concentrations. Conclusion: Interaction between genetic, endogenous and environmental factors is required to initiate Hashimoto's disease. As parvovirus B19 has been associated with autoimmune thyroid diseases, its possible role in the pathogenesis of Hashimoto's thyroiditis has not been studied. We investigated the potential role of B19 infection in inducing Hashimoto's thyroiditis. There was no significant difference between the patient and control groups in terms of ELISA and PCR outcomes but Parvovirus IgG positivity of %37,1 of patients indicates previous Parvovirus B19 infections may be one of triggers in the pathogenesis of Hashimoto's thyroiditis. Further prospective cohort studies with higher number of patients are required to help clarify association in the future. Keywords: Adolescent, Parvovirus B19, Hashimoto disease, autoimmune diseases
