Giriş ve Amaç: Alopesi Areata, nedeni tam olarak bilinmeyen, kronik otoimmun bir hastalıktır. İnsidansı %0,1-0,2 arasında değişir, yaşam boyu ortaya çıkma riski %1,7 olarak bildirilmiştir. Karakteristik lezyonu keskin sınırlı yuvarlak veya oval, skuamlı olmayan ve skatris bırakmayan, yama tarzında saç kayıplarıdır. Sadece saçlı deride değişik derecede saç kaybı ile giden formuna Alopesi Areata (AA), sadece saçlı derideki tam kayıpla giden formuna Alopesi Totalis (AT)ve tüm vücut kıllarında eş zamanlı kayıplarla ( kaş, kirpik dökülmesi gibi) giden formuna Alopesi Üniversalis (AÜ) denilmektedir. Tedavide topikal, intralezyonel ve sistemik steroidler, fototerapi, topikal minoksidil, siklosporin, kontakt duyarlandırıcılar ve topikal immunoterapiler kullanılmaktadır. Standart tedavilerden fayda görmeyen veya tolere edemeyen hastalarda topikal immunoterapi alternatif bir seçenektir. Çalışmamızda Alopesi Areata ve Alopesi Üniversalis hastalarında Skuarik Asit Dibutil Ester (SADBEile yapılan immunoterapi etkinliğinin değerlendirilmesi amaçlanmıştır. Yöntem: SADBE tedavisi ile immunoterapi uygulanan alanlarda periyodik olarak alerjik kontakt duyarlılık oluşturularak, deri eklerinin farklı noktalardan immunmodülasyonu ile o bölgede saç çıkışının sağlanması amaçlanmaktadır. Çalışmamızda 15 Ocak 2016-30 Nisan 2017 tarihleri arasında dermatoloji polikliniğimize başvuran, standart tedavilere dirençli AA veya AÜ/AT tanıları olan ve SADBE tedavisi alan 34 hastanın kayıtları incelendi. Hastalara ait demografik veriler, kötü prognostik faktörlerin varlığı ve SADBE tedavisine yanıtları retrospektif olarak değerlendirildi. Verilerin istatistiksel analizi İBM SPSS 21.0 programında yapıldı ve değişkenler için Pearson Ki kare, Fisher exact, Friedman, Mann-Whitney –U testi ve Kolmogorov Smirnov testleri kullanıldı. Bulgular: Çalışmaya alınan 34 hastanın 16'sı (%47,1) kadın, 18'i (%52,9) erkekti. Başvuru sırasında hastaların %67,6'sı AA, %32,4'ü AT/AÜ idi. Tedavi öncesi hastaların %76,5'i şiddetli hastalığa sahipken tedavi sonunda bu oran %29,4'e gerilemiştir. SADBE tedavisine hastaların %55,9'u yanıt vermiştir. Yanıt veren hastaların %47,4'ü kozmetik olarak kabul edilebilir düzeye ulaşmıştır. Hastaların tedavi öncesi Severity of Alopecia Tool (SALT) skorları ile tedavi sonrası SALT skorları arasında anlamlı bir fark olduğu saptanmıştır (p=0,034). Hastaların hastalık şiddeti ile SADBE tedavisine verilen yanıt arasında anlamlı bir ilişki saptanmıştır (p=0.006). Hastaların %55,9'unda yan etki olarak geçici eritem görülmüştür. Sonuç: Haftalık SADBE tedavisi dirençli AA ve AT/AÜ hastalarında etkin olması ve belirgin bir ciddi yan etkisinin olmaması nedeniyle tercih edilebilecek iyi bir seçenektir. Introduction: Alopecia Areata is a chronic, autoimmune disease of unknown cause.The incidence varies between 0,1-0,2% and the risk of life-long exposure is reported to be 1,7%. Characteristic lesion is sharp, round or oval, non-squamous and noncicatricial, patch-like hair loss. The patchy type of hair loss only on the scalp is called as Alopecia Areata (AA), full hair loss on the scalp called as Alopecia Totalis (AT) and simultaneous hair loss all body (such as eyebrows, eyelashes) is called Alopecia Universalis (AU). Topical, intralesional, systemic steroids, phototherapy, topical minoxidil, cyclosporine, contact sensitizers and topical immunotherapies are treatment options. Topical immunotherapy is an alternative option in patients who do not benefit or tolerate standard treatments. The aim of this study was to evaluate the efficacy ofSADBE treatmentin Alopecia Areata and Alopecia Universalis patients who had previously been treated with various topical and systemic treatments with no benefits in our dermatology outpatient clinic. Methods: Allergic contact sensitivity is created periodically in areas where immunotherapy is applied with SADBE treatment, and it is aimed to provide hair growth in that area by immunmodulation of skin appendages from different points. The records of 34 patients who were admitted to our dermatology outpatient clinic between 15 January 2016 and 30 April 2017 with the diagnosis of AA or AT / AÜ and treated with SADBE treatment were evaluated. Demographic datas, presence of poor prognostic factors and their response to SADBE treatment were retrospectively analyzed. Data were evaluated in İBM SPSS 21.0 program and Pearson Chi square, Fisher exact, Friedman, Mann-Whitney-U test and Kolmogorov Smirnov tests were used for the variables. Results: Of the 34 patients included in the study, 16 (47,1%) were female and 18 (52,9%) were male. 67.6% of the patients were AA, and19,4 % of the patients were AU / AT. Before the treatment, 76,5% of patients had severe disease and this rate decreased to 29,4% at the end of the treatment. Of the patients 55,9 % responded to SADBE treatment. İn the responding patients 47,4% reached a cosmetically acceptable levels. There was a significant difference between the Severity of Alopecia Tool (SALT) scores of the patients before and after the treatment (p = 0,034). A significant relationship was found between the disease severity and response to treatment (p = 0.006). In 55,9% of patients, transient erythema was seen as a side effect. Conclusion: Weekly SADBE treatment is a good option in patients with refractory AA and AT / AU because of its effectiveness and has no significant side effects.
