Transkateter arteriyel kemoembolizasyon, inopere hepatosellüler karsinomalı hastaların tedavisinde rutin olarak kullanılan bir yöntemdir. İlerlemiş karsinomalarda veya karsinomanın asıl arteriyel kaynağı olan arteria hepatica propria tıkanmış ise; ekstrahepatik kollateral arterler gelişebilir. Hem sağ hem de sol arteria phrenica inferior'lar (API), bu kollateral yollar arasında en sık ve önemli olanlardır. Ancak; bu arterlerin topografik anatomisi, birçok anatomi kitabı, atlası ve yapılmış çalışmalarda ayrıntılı olarak tarif edilmemiştir. API'ların orijinleri ve dağılımlarını tanımlamak amacıyla; İstanbul Üniversitesi İstanbul Tıp Fakültesi Anatomi Anabilim Dalı'nda bilateral olarak 26 fikse kadavra üzerinde çalışıldı. 3'ü (% 11.6) aorta abdominalis, 2'si (% 7.6) truncus coeliacus orijinli olmak üzere toplam 5 kadavrada, API'lar ortak bir kütükten çıkmaktaydı. Sağ API; 13 tarafta (% 25) aorta abdominalis, 2 tarafta (% 3.8) arteria renalis, 1 tarafta (% 1.9) truncus coeliacus ve 1 tarafta (% 1.9) arteria gastrica sinistra orijinliydi. Sol API; 9 tarafta (% 17.4) aorta abdominalis ve 6 tarafta (% 11.5) truncus coeliacus orijinliydi. 6 kadavrada (4 sağ, 6 sol tarafta), sol API'nın ramus posterior ve ramus ascendens'lerinin ayrı orijinlere sahip olduğunu gözlemlendi. API'ların verdiği dallar arasında; r. ascendens, r. posterior, aa. suprarenales superiores, a. suprarenalis media, özofageal, gastrik ve vena cava inferior'a giden dallar görüldü. Hepatosellüler karsinoma'ları besleyen ekstrahepatik arteriyel kollaterallerin yarısını oluşturdukları düşünüldüğünde, her iki API sadece anatomistler için değil, cerrahlar ve girişimsel radyologlar için de önem kazanmaktadır.
Transcatheter arterial chemoembolization is a common treatment for patients with nonoperable hepatocellular carcinoma. If the carcinoma is advanced or the main arterial supply, which is hepatic artery, is occluded; extrahepatic collateral arteries may develop. Inferior phrenic arteries (IPA), both right and left are the most frequent and important among these collaterals. Yet, the topographic anatomy of these arteries have not been described in detail in most anatomy textbooks, atlases and previous reports. In order to define the origins and distribution of the IPA, we studied 26 embalmed cadavers bilaterally at the Istanbul Faculty of Medicine, Department of Anatomy. The IPA?s originated as a common trunk in 5 cadavers; 3 (% 11.6) from abdominal aorta and 2 (% 7.6) from coeliac trunk. The right IPA originated from abdominal aorta in 13 sides (% 25), renal artery in 2 sides (% 3.8), coeliac trunk in 1 side (% 1.9) and left gastric artery in 1 side (% 1.9). The left IPA originated from abdominal aorta in 9 sides (% 17.4) and coeliac trunk in 6 sides (% 11.5). In 6 cadavers, regarding 4 right and 6 left sides, the ascending and posterior branches of the left IPA had different source of origins. The ascending, posterior, superior suprarenal, middle suprarenal, inferior vena caval, oesophageal and gastric branches of the IPA have been observed. As both IPA?s represent the half of all extrahepatic arterial collaterals to hepatocellular carcinomas, their anatomy gains importance not only for anatomists but surgeons and interventional radiologists as well.
