Amaç: Bu tez çalışması kapsamında; meme kanseri olan postmenopozal hastalarda
adjuvan hormonoterapide tercih edilen aromataz inhibitörlerinin nüks paterni ve sağkalım
ile vücut kitle indeksi arasındaki ilişki retrospektif olarak değerlendirildi ve aromataz
inhibitörlerinden letrozol ve anastrozolün vücut kitle indeksine göre sağkalıma etkisi
karşılaştırıldı.
Gereç ve Yöntem: Araştırmamızda hastanemiz onkoloji bölümünde postmenopozal meme
kanseri tanısı almış olan hastaların dosyalar retrospektif olarak değerlendirildi. Hastaların
yaş,menopoz yaşı, parite sayısı, aile öyküsü,sigara içme öyküsü, tanı tarihi, tanı anındaki
beden kitle indeksi, boyu, kilosu, histopatolojik tanısı, tümör büyüklüğü, aksiller lenf nodu
tutulumu ve sayısı, tümör derecesi, cerbB-2 overekspresyonu olup olmaması durumu,
hormon reseptör durumu (ER/PR pozitifliği), CA15-3 düzeyi, meme kanseri tanı anındaki
hastalık TNM evresi, lenfatik invazyonu,vasküler invazyonu gibi tümör özellikleri,
operasyon durumu, operasyon tipi, hastaya verilen adjuvan kemoterapi ve adjuvan
hormonoterapiler, radyoterapi verilip verilmediği, nüks gelişen hastalarda nüks tarihi, ölüm
zamanı, hastalıksız sağkalım ve genel sağkalım zamanı verileri kaydedildi.
Tanı anında metastatik olmayan ve hormon reseptörü pozitif adjuvan hormonoterapi
verilen 352 tane postmenopozal kadın hasta çalışmaya dahil edildi ve dosyaları retrospektif
olarak incelendi.
Çalışmanın sonlanım noktaları hastalıksız sağkalım ve genel sağkalım ve buna VKİ'nin
etkisi olarak belirlenmiştir. Beden kitle indeksi kategorileri Dünya Sağlık Örgütü'nün
kriterlerine göre düşük ağırlıklı ˂18,5 kg/m2; normal sınırlar 18,5-24,9 kg/m2; 1. derece
obezite 25-29,9 kg/m2; 2. derece obezite 30,0-39,9 kg/m2; 3. derece obezite >40 kg/m2
olacak şekilde belirlenmiştir. Biz hasta sayısı yetersiz olduğundan VKİ <25 kg/m², VKİ
25-29,9 kg/m², VKİ 30-40 kg/m², >40 kg/m² olmak üzere 4 gruba ayırdık.
Hastalar tanı anındaki vücut kitle indeksi, CA15-3 değeri, paritesi, aile öyküsü, sigara
kullanım öyküsü, hastalığın evresi, aldığı adjuvan hormonoterapiye göre sınıflandırıldı.

352 hastanın 56 tanesinde nüks gelişmiş olup bu hastaların hastalıksız sağkalım süreleri ay
olarak hesaplandı.
Bulgular: Nüks gelişen 56 hasta VKİ'ne göre gruplandırıldığında VKİ<25 kg/m² 7 hasta,
VKİ 25-29,9 kg/m² 23 hasta, VKİ 30-40 kg/m² 24 hasta, VKİ>40 kg/m² ise 2 hasta
mevcuttu.
Ex olan 56 hasta VKİ'ne göre gruplandırıldığında VKİ<25 kg/m² 5 hasta, VKİ 25-29,9
kg/m² 26 hasta, VKİ 30-40 kg/m² 22 hasta, VKİ>40 kg/m² ise 4 hasta mevcuttu.
Anastrozol verilen hastalar arasında sırasıyla ortanca HSK ve GSK süresi 53 ay (%95 CI;
31,00-76,50) ve 59 ay (%95 CI; 37,75-81,25) saptandı. Sırası ile 2 yıllık HSK ve GSK
oranı %94 ve %96 olarak bulunurken; 5 yıllık HSK ve GSK oranı %81 ve %89 olarak
bulundu.
Letrozol verilen hastalar arasında ortanca HSK ve GSK süresi 65 ay (%95 CI; 37,00-
94,00 ) ve 68 ay (%95 CI; 41,00-95,00) saptandı. Sırası ile 2 yıllık HSK ve GSK oranı
%95 ve %98 olarak bulunurken; 5 yıllık HSK ve GSK oranı %89 ve %86 olarak bulundu.
Anastrozol ve letrozol verilen gruplar arasında HSK ve GSK açısından bakılan istatiksel
analizde anlamlı farklılık saptanmadı. Anastrozol ve letrozol verilen hastalarda HSK ve
GSK sürelerinin benzer olduğu görüldü .
Sonuçlar: Çalışmamızda elde ettiğimiz veriler ışığında hormon reseptörü pozitif erken
evre meme kanserli hastalarda aromataz inhibitörlerinden anastrozol ve letrozol verilen
grupta vücut kitle indekslerinin arttıkça hastalıksız sağkalım süresinin değişmediği ayrıca
tanı anındaki VKİ profilinin hastalıksız sağkalım süresini etkilemediği tespit edildi.

Introduction: Within the scope of this thesis; The recurrence pattern of the preferred
aromatase inhibitors in adjuvant hormone therapy in postmenopausal patients with breast
cancer and the relationship between survival and body mass index were evaluated
retrospectively and the effects of letrozole and anastrozole on aromatase inhibitors
according to body mass index were compared.
Material and Method: In this study, the files of patients with postmenopausal breast
cancer diagnosed in the oncology department of our hospital were evaluated
retrospectively. Patients' age, menopause age, parity number, family history, smoking
history, date of diagnosis, body mass index at the time of diagnosis, height, weight,
histopathologic diagnosis, tumor size, axillary lymph node involvement and number, tumor
grade, cerbB-2 overexpression absence, hormone receptor status (ER / PR positivity),
CA15-3 level, breast cancer at the time of diagnosis TNM stage, lymphatic invasion,
vascular invasion, tumor characteristics, operation status, type of operation, adjuvant
chemotherapy and adjuvant hormones given to the patient, radiotherapy data on whether
recurrence, recurrence date, time of death, disease-free survival and overall survival time
were recorded.
352 postmenopausal female patients who were given non-metastatic and hormone
receptor positive adjuvant hormone therapy at the time of diagnosis were included in the
study and their files were reviewed retrospectively.
The endpoints of the study were disease-free and overall survival and the effect of BMI
on this. As the number of patients was insufficient, we divided the BMI into 4 groups: <25
kg / m², BMI 25-29.9 kg / m², BMI 30-40 kg / m² and > 40 kg / m².
The patients were classified according to body mass index, CA15-3 value, parity, family
history, smoking history, stage of the disease and adjuvant hormone therapy.
Recurrence occurred in 56 out of 352 patients and disease-free survival was calculated
in months.

Results: When 56 patients with recurrence were grouped according to BMI, there were 7
patients with BMI <25 kg / m², 23 patients with BMI 25-29.9 kg / m², 24 patients with
BMI 30-40 kg / m² and 2 patients with BMI > 40 kg / m².
When 56 patients with ex were grouped according to BMI, there were 5 patients with
BMI <25 kg / m², 26 patients with BMI 25-29.9 kg / m², 22 patients with BMI 30-40 kg /
m² and 4 patients with BMI > 40 kg / m².
Among patients given anastrozole; The median duration of DFS and OS was 53 months
(95% CI; 31.00-76.50) and 59 months (95% CI; 37.75-81.25). The 2-year DFS and OS
rates are 94% and 96% respectively; The 5-year DFS and OS rates were 81% and 89%.
Among patients given letrozole; The median duration of DFS and OS was 65 months
(95% CI; 37.00-94.00) and 68 months (95% CI; 41.00-95.00). The 2-year DFS and OS
rates are 95% and 98% respectively; The 5-year DFS and OS rates were 89% and 86%,
respectively.
No statistically significant difference was found between the anastrozole and letrozole
groups in DFS and OS. The duration of HSK and GSK were similar in patients treated with
anastrozole and letrozole.
Conclusion: In the light of the data obtained in our study, it was found that the diseasefree survival time did not change as the body mass indexes increased in aromatase
inhibitors anastrozole and letrozole group in patients with hormone receptor positive early
breast cancer, and that BMI profile at the time of diagnosis did not affect disease-free
survival.
