Bu çalışmada konjuge pnömokok aşısının kullanımı ile birlikte nazofaringeal taşıyıcılığın en sık görüldüğü 0-5 yaş arası sağlıklı çocuklardan alınan nazofaringeal sürüntü örneklerinde Streptococcus pneumoniae varlığı araştırılmış ve izole edilen suşlarda serogrup/serotip dağılımı ve antibiyotiklere direnç oranları belirlenmiştir.İstanbul Üniversitesi İstanbul Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Sosyal Pediatri Bilim Dalı'ndan takipli 0-5 yaş arası 150 sağlıklı çocuktan nazofaringeal sürüntü örnekleri alındı. 2,5 µg/ml gentamisin içeren % 5 koyun kanlı agara ekim yapılarak Streptococcus pneumoniae varlığı araştırıldı. İzole edilen Streptococcus pneumoniae suşlarının çeşitli antibiyotiklere direnç oranları disk difüzyon yöntemi ile araştırıldı ve lateks aglütinasyon yöntemi kullanılarak serogrup/serotiplendirme yapıldı.Sağlıklı 150 çocuğun 42'sinde pnömokok suşu izole edildi ve taşıyıcılık oranı % 28 olarak saptandı. 20'si serogrup 19 (% 47), 7'si serogrup 6 (% 16), 5'i serogrup 23 (% 12), 4'ü serogrup 15 (% 9), 2'si serogrup 10 (% 4), 2'si serogrup11 (% 4) ve 1'i serogrup 17 (% 2) olarak belirlenirken bir suş tiplendirilemedi. En sık izole edilen suşlar serotip 19, 6 ve 23 olarak belirlendi.Antibiyotik duyarlılık deneyi sonucunda iki pnömokok suşu oksasiline duyarlı olarak bulundu ve diğer pnömokok suşları için yapılan penisilin E-test sonucuna göre yüksek ve orta düzey penisilin direnci sırasıyla % 4,7 ve % 69 olarak saptandı.Eritromisin, klindamisin, trimetoprim-sulfametoksazol, tetrasiklin ve kloramfenikol direnç oranları sırasıyla % 76 (n=32), % 59 (n=25), % 73 (n=31), % 62 (n=26) ve % 9 (n=4) olarak bulunurken vankomisin, levofloksasin ve telitromisine direnç saptanmadı. En dirençli serogruplar yine serogrup 19, 6 ve 23 olarak belirlendi.
In this study, it was aimed to detect the presence of Streptococcus pneumoniae in nasopharyngeal swabs of 0-5 years old healthy children in whom the nasopharyngeal carriage was most often detected after use of the conjugated pneumococcus vaccine and to detect serogroup/serotype distribution and the susceptibilities to antibiotics of the strains isolated.Nasopharyngeal swabs were taken from 150 healthy children between 0-5 years old who were followed in Istanbul University Istanbul Medical Faculty Pediatrics Department Social Pediatri Division. S. pneumoniae was investigated by inoculating the samples onto 5 % sheep blood agar containing 2,5 µg/ml gentamycin. Antibiotic susceptibilities of the strains isolated were investigated by disc diffusion and E-test methods and serogroup/serotyping was done by agglutination method.S. pneumoniae was isolated from 42 of 150 healthy children and carriage rate was detected to be 28 %. 20 (47 %) belonged to serogroup 19, 7 (16 %) belonged to serogroup 6, 5 (12 %) belonged to serogroup 23, 4 (9 %) belonged to serogroup 15, 2 (4 % ) belonged to serogroup 10, 2 (4 %) belonged to serogroup 11 where as one strain could not be typed. The most frequently isolated serotypes were detected to be serotypes 19, 6 and 23.According to antibiotic susceptibility test, two pneumococcus isolates were found to be susceptible to oxacillin and according to the penicillin E-test carried out for the other pneumococcus isolates, high level penicillin resistance rate and intermediate level penicillin resistance rate was found to be 4,7 % and 69 % respectively.The resistance rates to erythromycin, clindamycin, trimethoprim-sulfamethoxazole, tetracycline and chloramphenicol were found to be 76 % (n=32), 59 % (n=25), 73 % (n=31), 62 % (n=26) and 9 % (n=4) where as no resistance to vancomycin, levofloxacin and telithromycin was detected. Serogroup 19, 6 and 23 was found to be the most resistant serogroups.
