Skolyoz tedavisinde deformite korreksiyonu, sistemin kalıcı tespitini sağlayacak vida, kanca gibi elemanların roda tutturulup rod üzerinden komprese, distrakte, derote edilmesi veya translasyon yapılması ile sağlanmıştır. Deformiteyi düzeltmek için ideal olan, yerleştirilen sistem elemanlarını ve bağlı olduğu kemik bölgesini aşırı zorlamadan, zarar vermeden gerekli düzeltme işlemlerini gerçekleştirmektir.Deformite düzeltmede 2006 yılından beri Posteriordan intraoperatif Geçici Gerdirme (PGG) adlı bir teknik kullanmaktayız. Yöntemde ilizarov gerdirme aleti, özel tasarlanmış distal ucu rod kalınlığında olan ilizarov teli kullanılmaktadır. Teknik her tür pedikül vida sistemiyle kullanıbilmektedir. PGG yöntemiyle istenilen ölçüde korreksiyon yapılabilmektedir.Kliniğimizde son 4 yılda 18 hastaya bu yöntem ile posterior enstrumantasyon ve fuzyon yapılmıştır. Hastaların yaş ortalaması 16.05 tir. Frontal planda %68.8'lik korreksiyon elde edilmiştir. SRS-30 skor ortalamamız 3.83'tür.Posteriordan intraoperatif geçici gerdirme yönteminin her tür pedikül vida sistemiyle kullanılabilmesi ve istenilen ölçüde korreksiyona izin vermesi nedeniyle uygun bir cerrahi teknik olduğunu düşünmekteyiz.
Deformity correction in treatment of scoliosis has been done by binding permanent fiksations of screw, hook like spinal instruments to the rod and then by making compression , distraction, derotation or translation on the rod.The ideal correction for deformity is that the necessary correction processes should be done without forcing too much and not damaging placed pedicular system enstruments and their bone areas.Temporary Intraoperative Posterior Stretching(TIPS) technique has been used in our clinic since 2006. Ilizarov wire, whose distal diameter is the same as the rod , and streching tool have been used in the technique. This technique can be used in all kind of pedicul screw systems. TIPS permits correction of the deformity in intended measurement.18 patients were treated with posterior instrumentation and fusion by using this tecnique in our clinic in the last 4 years. The average age of the patients is 16.05 . The correction of 68.8% in the frontal plane is acquired. The avarege score of the SRS-30 is 3.83.Since we can use all kinds of pedicular screw systems with this technique and it permits intended measurement of correction, we believe it is useful and appropriate surgical technique.
