105 ÖZET Çiğneme basıncının protez vasıtasıyla mukoza ve kemik üzerine dengeli dağılması istenilmektedir. Fonksiyon sırasında gerilim yoğunlaşmaları ağrıya, destek dokularda ülserlere, rezorpsiyon hızının artmasına sebep olabilmektedir. Yumuşak astar maddelerinin kullanımının gerilme lokalizasyonuna ve nümerik değerlerine etkisi sonlu elemanlar gerilme analizi metodu ile bu çalışmada araştırılmıştır. Sonlu elemanlar gerilme analizinde (ANSYS5.4) iki farklı (mandibular kanallı ve kanalsız) matematik modellerden edilen asal birim uzama değerleri strain gauge gerilme analizinden edinilen asal birim uzama değerleri ile kıyaslanmıştır. Mandibular kanallı matematik modelin sonlu elemanlar gerilme analizi sonuçlan strain gauge fiziksel deneyi ile tutarlı bulunmuştur. Çalışmanın devamı bu matematik model üzerinde sürdürülmüştür. Bu çalışmada akrilik kaideli ve yumuşak astar maddesi (Molloplast- B) ile astarlanmış tam protezlere alt çeneyi kapatan kasların uyguladığı kuvvetlerin vektörel bileşenlerinin simülasyonu sonlu elemanlar gerilme analizi metodunda gerçekleştirilmiştir. Mandibula, yumuşak doku, ve alt tam protezin eşdeğer gerilme değerleri ve yoğunlaşma bölgeleri tespit edilmiştir. Her iki protezde de kaide plağmdaki en yüksek eşdeğer gerilme orta keserlerin lingualine lokalize olmuştur. Akrilik kaide plaklı tam protez yumuşak astar maddesi ile astarlandığında, kaidenin en yüksek eşdeğer gerilme değeri % 15 oranında artmaktadır. Bu durum yumuşak astar maddeleri ile astarlanan tam protezlerde orta hat kırılmaları ile klinikte daha sık karşılaşılmasmı açıklamaktadır. Yumuşak astarlı tam protezin mukozasında ise görülen en yüksek eşdeğer gerilme lokalizasyonu kretin106 lingual tarafında ve 2. Azı dişi hizasında oldudu saptanmıştır. Akrilik tam protezin mukozasında görülen en yüksek eşdeğer gerilmelerin lokalizasyonu kretin lingual tarafında ve 1. Azı hizasında bulunmuştur. Yumuşak astarlı tam protezin mukozasında ise görülen en yüksek eşdeğer gerilme lokalizasyonu kretin lingual tarafında ve 2. Azı dişi hizasında olduğu saptanmıştır. Ayrıca yumuşak astar maddelerinin kullanımı destek mukozada en yüksek eşdeğer gerilme değerini %22 oranında azaltmıştır. Akrilik kaideli ve yumuşak astarlı tam protezlerin görülen en yüksek eşdeğer gerilmelerin lokalizasyonu alveol kretin tepesinde ve 3. Azı hizasında bulunmuştur. Ayrıca yumuşak astar maddelerinin kullanımı destek kemikte en yüksek eşdeğer gerilme değerini %26 oranında arttırmıştır. Fakat yumuşak astar maddelerinin kullanımı mandibular foramenin distalindeki destek kemikte eşdeğer gerilme değerini % 21 oranında azaltmıştır.
107 SUMMARY A balanced distribution of the masticatory forces on the mucosa and bone via the denture is required; since stress concentrations may produce pain or ulceration at the supporting tissues and resorption of the alveolar bone under complete dentures during the chewing function. In this study, the effect of using a soft liner in the mandibular denture base, on stress distribution on the alveolar mucosa and the bone is investigated by Finite Element stress analysis Method (FEM). Principle strains for two different mathematic models of edentulous mandible ( with mandibular canal or without mandibular canal) calculated by the finite element method (ANSYS5.4), are compared with those found by the experimental method. It has been seen that, the FEM results obtained for the mandible with mandibular canal, are in compliance with the experimental results which are achieved using the strain-gauge stress analysis techniques. And the study is built on this model. The physiological muscle loads acting on complete dentures with conventional denture base (acrylic base) and soft liner (Molloplast-B) are simulated in this study by the finite element stress analysis method. Highest equivalent stresses of the mandibula, soft tissue and lower complete denture, and the regions where these values are concentrated are established. Highest equivalent stresses at the denture base are localised on the lingual surfaces of the lower central incisors for both of dentures: The conventional and soft lined mandibular complete dentures. Highest equivalent stresses at the denture base increases by 15% when relined with soft liner. This explains the frequent occurrence of fractures at the midline108 of dentures clinically. For the complete dentures with conventional base, the highest equivalent stresses at the soft tissue under the lower base were localised at the region corresponding to the lingual surface of the first molar artificial teeth. For the complete dentures with soft liner, the highest equivalent stresses at the soft tissue under the lower base were localised at the region corresponding to the lingual surface of the second molar artificial teeth. Additionally, by using soft liner, the highest equivalent stresses at the supporting mucosa are decreased by 22%. The highest equivalent stresses at the supporting bone for both conventional denture base and soft liner at the complete lower dentures were localised on crest of the alveolar ridge under the base of the lingual surface of the third molar artificial teeth. Besides, usage of soft liner, increased the highest equivalent stresses at the supporting bone by 26%. But, the equivalent stresses on the bone distal of mental foramen under the soft lined denture base were decreased at a ratio of21%.
