Amaç : Bu çalışmanın amacı akromegali sebebi ile takip edilen hasta grubunda ilk
başvuru şikayetleri , hangi hekime başvurdukları , tanıyı hangi hekim tarafından
konulduğunu,tanıda gecikme zamanı , tanı almadan önce geçirdiği ameliyatlar, mortalite
ve ölüm sebeplerin tespit edilmesidir.
HASTA SEÇİMİ VE YÖNTEMİ
Bu çalışmaya ; İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi İç Hastalıkları Anabilim
Dalı, Endokrinoloji-Metobolizma ve Diyabet Bilim Dalı polikliniğinde 1998-2014 yılları
arasında akromegali tanısı konulup takip edilen 500 hasta telefonla arandı. Toplam 500
hastadan 335 hastaya ulaşıldı , 165 hasta çalışma dışı bırakıldı.
BULGULAR
192 Kadın ( % 57,3) , 143 erkek (% 42,7), yaş ortalamaları 48.6 ±
12 yıldır. Tanıda gecikme ortancası 24 ay ( 1 hafta- 180 ay) olarak saptandı. Yirmi iki
hasta vefat ettiği öğrenildi (11 Kadın , 11 Erkek ). Ölüm nedenleri : 3 hasta Miyokard
enfarktüsü(%13,6 ), 3'ü kalp yetersizliği (%13,6) , 2 'si Akciğer kanseri(%9,1) , 2' si kolon kanseri (%9,1), 2'si mide kanseri(%9,1), 2 'si Lösemi (%9,1), 2'si septik şok(%9,1) , 2 'si serebrovasküler hastalık (%9,1) , 1'i Akciğer ödemi (%4,5), 1'i hasta nöroendokrin Tm (%4,5) , 1'i primeri bilinmeyen kemik metastazı(%4,5) , 1 hastanında ölüm nedeni bilinmiyor(%4,5). Doktora başvurma şikayetleri akral büyüme %32.6 'sinde , baş ağrısı %26.2'sinde , herhangi yakınması olmayan %32.6'sinde, görme bozukluğu
%9.3' saptandı. Hastaların ilk başvurduğu hekimler sırasıyla genel dahiliye (%29.4) ,
nöroşirürji uzmanı (%11.8), nörolog (%11.2), endokrinolog (%8.3) ve akromegali en
çok endokrinolog 175 (%55.9) , nöroşirürji 72 (% 23 ), genel dahiliye 43 (%13.8 ),
nöroloji 9 (% 2.9 ) hekimler tarafından tanı konuldu. Baş boyun cerrahisi
40(%12.8) , burun ameliyatı 29 (%9.3), tiroidektomi 20(%6.4) , %5.1), karpal tünel
ameliyati 15 (%4.8), tonsillektomi 11 (%3.5) , kolesistektomi 6( %1.9).
Ameliyat ve tanı arası sürenin ortancası 48 ay (1- 420 ay) olarak saptandı ancak
170(%54.3) hastada da tanı öncesi herhangi bir ameliyat geçirmemiş.
Sonuç
Akromegalinin geniş komorbidite nedenleriyle ilişkilidir ve sıklıkla genel dahiliye
uzmanı , nöroşirurji uzmanı ve nöroloji uzmanına başvuryor. En çok tanı öncesi baş
boyun ameliyati geçiriyorlar. Akromegali hastalarında erken tanı koymak morbidite ve
mortalitenin azaltmasına sebep olur.

Objective:To evaluate the initial complaints, detailed history and surgical procedures prior to the first diagnosis in acromegalic patients.
Method:Three hundred thirty five out of 500 acromegaly patients who were under follow-up at University of Istanbul, Cerrahpasa Medical Faculty, Endocrinology and Metabolism outpatient clinic between 1998 and 2014 were included. Patients were asked via phone call or face to face interviews about their initial complaints, the physician they were admitted to first, the interval from onset of symptoms to diagnosis, and the previous surgeries. The surgery types and the possible relation of the surgery with acromegaly were evaluated.
Results:This study included 335 participants; 192 women and 143 men. The mean age was 48.6±12.0 years. Twenty one of them were dead. The frequent complaints of the patients were acral growth of hands and feet (32.6 %), headache (26.2%), visual defects (9.3%), menstrual irregularities (8%), and coarsened facial features (7%). The first physician the patients were admitted to was as follows; internist (29.4%), neurosurgeon (11.8%), neurologist (11.2%), ophthalmologist (8%), and ear nose throat specialist (6.1%).
Initial diagnosis was made by an endocrinologist (55%), neurosurgeon (23%), internist (13.7%), neurologist (2.9%).The median diagnosis delay time was 40 months.
Almost half of the patients had undergone an operation prior to diagnosis (45.7%). In total, 33.3% of the surgeries were related to acromegaly complaints. The most common operations were as follows; head and throat surgery (12.8%), nose surgery (9.3%), thyroidectomy (6.4%), carpal tunnel surgery (4.8%).
Conclusion:Acromegaly patients were mostly referred to internists because of co-morbidities, to ear nose throat specialist for deformities and to neurosurgeons due to the mass effect symptoms, Most frequently they had undergone head and neck surgery prior to diagnosis. Therefore, it is important that medical staff be aware of the clinical aspects of acromegaly which causes such prominent physical changes. |
