AMAÇ: Çalışmamızın amacı allogeneik hematopoietik kök hücre nakli (HKHN) sonrası nüks saptanan vakalarımızda donör lenfosit infüzyonu (DLİ) sonuçlarını ve bu sonuçlara etkili olabilecek faktörleri değerlendirmektir. HASTALAR VE YÖNTEM: Cerrahpaşa Tıp Fakültesi İç Hastalıkları Anabilim Dalı, Hematoloji Bilim Dalı Kök Hücre Nakil Ünitesi'nde 1993-2017 yılları arasında HKHN yapılan ve dış merkezde allogeneik HKHN yapılmış olup nüks saptanan hastaların dosyaları retrospektif olarak incelendi. Çalışmaya 57 hasta alındı. Hastaların demografik özellikleri, birincil tanıları, nüks tipi, nüks sonrası tercih edilen tedavi yöntemi, DLİ yapılan hastalarda CD3+ hücre dozu, zamanı, DLİ sonrası komplikasyonlar ile hasta ve donör arasındaki insan lökosit antijenleri (HLA), ABO uyumu, yakınlık derecesi kaydedilmiştir. Hastalar DLİ alan ve almayanlar olarak 2 grupta karşılaştırıldı. DLİ alanlar da kendi arasında verilen hücre sayısına göre karşılaştırıldı. BULGULAR: 30 hastaya (%52,6) DLİ verildiği saptandı. DLİ ardından remisyona giren ve girmeyen hastaların toplam CD3 hücre dozu açısından anlamlı farkları yoktu(ki-kare: 0,16 p=0,553>0,05). Tek seans veya 1'den fazla DLİ uygulaması alanlar arasında tedavi yanıtı açısından anlamlı fark yoktu(ki-kare:0,001 p=0,671>0,05). DLİ verilenlerin 5'inde (%17,9) remisyon yanıtı alındı. Remisyon oranlarını ve nüks sonrası sağkalımı etkileyen faktörlere bakıldığında hastalık tipi, greft-versus-host-hastalığı (GVHH) gelişimi, hastanın ve donörün viral seroloji durumu, hastanın dönor ile HLA ve ABO uyumu, yakınlık derecesi, verilen T hücre dozu ve seansı arasında anlamlı ilişki görülmedi (p>0.05). SONUÇ: Şimdiye kadar yapılan çalışmalar sonucunda DLİ tedavisi elimizdeki en güçlü silahtır, fakat ideal zamanlama ve dozu bilinmemekte, standart bir klinik uygulama bulunmamaktadır. DLİ mekanizmasının net anlaşılabilmesi ve standart bir klinik uygulama belirlenebilmesi için daha geniş hasta gruplarında çalışma yapılması gerekmektedir. OBJECTIVES: The aim of our study is to evaluate the outcomes of our donor lymphocyte infusions (DLIs) and the factors that are likely to be effective on these outcomes in cases of relaps after allogeneic hematopoietic stem cell transplantation (HSCT). PATIENTS AND METHOD: Files of 621 patients who were recepients of HSCT from 1993 to 2017 in Cerrahpaşa Medical Faculty Internal Medicine Department Hematology Section Stem Cell Transplantation Unit and patients who had undergone HSCT in the external center with relaps were also retrospectively examined. 57 patients were included in the study. Demographic characteristics of patients, primary diagnoses, treatments before transplantation, HSCT time, complications after HSCT, relapse type, preferred treatment method after relapse, CD3 + cell dose in patients with DLI, complications after DLI, human leukocyte antigen (HLA) and ABO compatibility between patient and donor, the donor type was recorded. Patients were compared in two groups as given DLI patients and not given DLI patients. DLI receivers were also compared according to the number of cells given between them. RESULTS: DLI was given to 30 patients (52.6%). There was no significant difference in the total CD3 + cell dose of the responder and non-responders (chi-square: 0,16 p = 0,553> 0,05). There was no significant difference in treatment response between patients who receive DLI one session or more than one.(chi-square: 0,001 p = 0,671> 0,05). Five of the DLI recipients (17.9%) entered the remission. When we examined the factors affecting the remission rates and survival after relaps in patients with DLI there was no significant relationship between the type of disease, graft-versus-host-disease (GVHD) progression after DLI, viral serology status of the patient and donor, HLA and ABO compliance, donor type, T cell dose and the sessions given to patients receiving DLI in our study (p>0.05). CONLUSIONS: As a result of studies up to now, DLI therapy is the most powerful weapon available, but the ideal timing and the dose are unknown, there is no standard clinical practice. It is necessary to study in larger patient groups in order to be able to understand the DLI mechanism clearly and to determine a standard clinical practise.
