Amaç : Akcier kanseri, tedavisi zor ve mortalitesi yüksek bir hastalıktır. Onkolojik
tedavi sonrası hastalarda nüks, progresyon ve çeitli medikal sorunlar sıklıkla ortaya
çıkmaktadır.
Çalımamızda akcier kanseri hastalarında onkolojik tedavi sonrasında saptanan
medikal sorunların belirlenmesi amaçlandı.
Metod: Çalımada Cerrahpaa Tıp Fakültesi Göüs Hastalıkları Anabilim Dalında
takipli Ocak 2012 tarihinden sonra histopatolojik olarak kesin akcier kanseri tanısı
konmu veya tedavisi bitmi 40 hastanın dosya verileri retrospektif olarak incelenerek
hastaların ya, cinsiyet, sigara kullanım miktarı, komorbid hastalıkları, akcier kanseri
tanı tarihleri, aldıkları tedavi rejimi, tedavi süreleri ve tedavi sonrasında saptanan
medikal sorunlar kaydedildi. statistiksel analiz için SPSS 20.0 ( Statistical Package for
Social Sciences, for Windows, USA) programı kullanıldı.
Bulgular: Hastaların 36?sı (%90) erkek, 4?ü (%10) kadın olup ya ortalaması 63.5± 8.8
yıl idi. 38 hasta (%95) sigara içmiti. Sigara içen hastaların ortalama sigara içme
miktarı 48.7± 22.1 paket-yıl idi. Hastaların 30?unda (%75) komorbidite bulunmaktaydı.
Komorbidite sıklıına baktıımızda 14 (% 35) olguda hipertansiyon, 12 (% 30) olguda
KOAH, 8(% 20) olguda koroner arter hastalıı, 6 (%15) olguda DM, 3 (%7,5) olguda
akcier tüberkulozu, 3 (%7,5) olguda hiperlipidemi, 2 (%5) olguda aritmi, 2 (%5)
olguda KKY, 1 (%2,5) olguda SVO ve 1 (%2,5) olguda KBY vardı. 17 hasta (%42,5)
adenokanser, 12 hasta (%30) skuamoz hücreli kanser, 9 hasta (%22.5) KHAK, ve 2
hasta (%5) tanımlanamayan KHDAK tanısı almıtı.
Kemoterapi ve radyoterapi tedavi yanıtlarına baktıımızda; 5 hastada tam yanıt, 13
hastada kısmi yanıt, 14 hasta yanıtsız ve 5 hastada progresyon saptandı. KHAK
olguların KHDAK?e göre onkolojik tedavi yanıtları daha iyiydi. ( p= 0.044)
Adenokanser olgularında onkolojik tedavi yanıtı skuamoz hücreli kansere göre daha
iyiydi. (p=0.131)
Hastalar onkolojik tedaviden ortalama 94,1 ± 72 gün sonra medikal sorun nedeniyle
hastaneye bavurdu. Hastaların tedavi sonrası hastaneye bavurma semptomlarından en
sık nefes darlıı görüldü. 21 hasta (%52,5) nefes darlıı, 12 hasta (%30) öksürük, 8
hasta (%20) balgam, 8 hasta (%20) ate, 8 hasta (%20) halsizlik, 6 hasta (%15) arı, 6
hasta (%15) genel durum bozukluu, 4 hasta (%10) hemoptizi, 3 hasta (%7,5) bilinç
bozukluu, 3 hasta (%7,5) bacaklarda ilik, 2 hasta (%5) yürüme zorluu, 1 hasta
(%2,5) konuma bozukluu, 1 hasta (%2,5) denge bozukluu, 1 hasta (%2,5)
konvülziyon, 1 hasta (%2,5) hemipleji ve 1 hasta (%2,5) bacakta uyuukluk ikayetleri
ile hastaneye bavurdu.
Onkolojik tedaviden sonra medikal sorun nedeniyle hastaneye bavuran hastalarda en
sık hastalık progresyonu (%32,5), nüks (%30) ve pnömoni (%25) saptandı. Tedaviye
yanıt alınan olgularda nüks ortalama 3,7 ± 2 ay sonra gözlendi. Tedaviye yanıt
alınamayan olgularda hastalık ortalama 1,9 ± 1,8 ay içinde progrese etti. Nüks ve
progresyon en sık akcierde geliti. Kitleye yönelik küratif radyoterapi uygulanan
hastalarda, uygulanmayanlara göre nüks görülme oranı daha azdı. (p=0,023) KHAK
hastalarında KHDAK?e göre nüks daha sık saptandı. (p =0.6)
Hastaların ortalama sa kalım süreleri 17,5 ay (%95 güven aralıı 14,9-20,1) medyan
sakalım süreleri 17 ay (%95 güven aralıı 12,7 - 21,2) olarak saptandı.
Sonuç: Akcier kanseri hastalarında onkolojik tedavi sonrası en sık saptanan medikal
sorunlar tümörün nüks ve progresyonudur. Onkolojik tedavi sonrası olgular ortalama
94,1 ± 72 gün sonrası medikal nedenlerle hastaneye bavurmaktadırlar. Tedavisi
tamamlanan akcier kanseri olgularında tedaviye yanıtın deerlendirilmesinden sonraki
ilk klinik deerlendirme ve radyolojik inceleme üç ay içinde yapılmalıdır. Olgunun
tedaviye yanıtına göre bu süre daha erkene çekilebilir.

Introduction:Lung cancer is a difficult to treat disease with high mortality. After
oncological treatment patients often arise relapse, progression of disease or various
medical complications. The goal of our study was to describe and assesss the medical
complications detected after oncological treatment of lung cancer.
Methods: In this retrospective study we reviewed the charts of 40 patients who were
diagnosed with lung cancer by histopathological examination or finished the
oncological treatment after January 2012 and followed by the Department of Pulmonary
Diseases at Cerrahpasa Faculty of Medicine. Data regarding age and gender
distribution, smoking history, comorbidities, diagnosis date, treatment regimen, duration
of the treatment and medical complications detected after oncologic treatment was
collected. Statistical analysis was done using SPSS 20.0 (Statistical Package for Social
Sciences, for Windows, USA) program.
Findings: Average age of the patients was 63.5± 8.8; 36 (90%) were males, 4 (10%)
females. 38/40 (95%) were smokers with a mean duration of 48.7+/-22.1 pack-years.
30% of the patients had at least one medical comorbidities, including 14 (35%) with
hypertension, 12 (30%) with COPD, 8 (20%) with coronary artery disease, 6 (15%)
with diabetes mellitus, 3 (7,5%) with pulmonary tuberculosis, 3 (7,5%) with
hyperlipidemia, 2 (5%) with arrhythmia, 2 (5%) with congestive heart failure, 1 (2,5%)
with stroke and 1 (2,5%) with chronic renal failure. . Histopathologic examination
revealed that 17 (42.5%) of the cases were adenocarcinoma, 12 (30%) squamous cell
carcinoma, 9 (22.5%) small cell carcinoma and 2 (5%) cases of unidentified NSCLC.
Regarding the response rates of chemotherapy and radiation, 5 patients had complete
response, 13 of them partial response, whereas 14 patients did not respond to treatment
and 5 patients demostrated progression. n SCLC patients oncological treatment
response were better compared with NSCLC patients. (p= 0.044) n adenocarsinoma the
patients oncological treatment response where better compared with squamous cell
carsinoma too. (p=0.131)
After the cancer treatment, most patients were re-admitted to the hospital within an
average of 94.1+/-72 days with medical complications. 21 (52,5%) patients presented
with shortness of breath, 12 (30%) with cough, 8 (20%) with increased sputum, 8 (20%)
with fever, 8 (20%) with fatigue, 6 (15%) patients with pain symptoms, 6 (15%) with
other constitutional symptoms, 4 (10%) with hemoptysis, 3 (7.5%) with altered mental
status, 3 (7.5%) with lower extremity edema, 2 (5%) with difficulty walking and one
each (2.5%) with aphasia, seizures, imbalance, hemiplegia and leg paresthesia,respectively.
Among these patients the etiology of the symptoms was found to be most often cancer
progression (32.5%), relapse (30%) and pneumonia (25%). In the patients that
responded to oncological treatment, relapse was seen on average after 3,7 ± 2 months.
In the cases that did not respond to oncological therapy, progression was noticed within
1,9± 1,8 months. Both relapse and progression of the cancer was more frequently seen
in the lungs. Relapse was seen less frequently in the patients treated with curative
chemotherapy (p=0.023), but more often in small cell cancer cases. (p=0.6).
Overall, average duration of survival of our patients was 17.5 months (95% Confidence
Interval 14.9-20.1), whereas median duration of survival was 17 months (95%
Confidence Interval 12.7-21.2)
Conclusions: In lung cancer patients after oncological treatment, relapse and
progression are the most frequent complications that follow oncological therapy. These
patients have a high re-admission rate due to complications with an estimated duration
until re-admission of 94.1+/-72 days. All cases that complete therapy should be reevaluated
clinically and radiologically within three months after the evaluation of
treatment response. These period can be reduced based on the individual response to
therapy. |
