Akrilik maddesinin laboratuvar işlemleri sırasında ideal koşullar sağlansa bile malzemenin doğasına bağlı olarak polimerizasyonu boyunca ve sonrasında genleşmeler ve büzülmeler oluşmaktadır. Akrilik reçinedeki bu değişiklikler protezin bütününü etkileyerek boyutsal değişikliklere sebep olurlar. Boyutsal değişikliklere neden olan sebeplerden bir tanesi de protezin genel veya yerel sıcaklıklara maruz kalması olabilir. Protezi etkileyen bölgesel sıcaklık artışları protezin tesviyesi ve polisajı sırasında, genel sıcaklık artışları da ağza alınan sıcak yiyecek ve içecekler veya protezlerin sıcak su içerisinde bekletilmeleri sonucu meydana gelir. Bu doktora çalışmasında akrilikten yapılmış alt tam protezlerin sıcaklığa maruz kaldıklarında oluşan boyutsal değişiklikler ve bu oluşan değişikliklerin nedenleri araştırılmıştır. Doktora çalışması sekiz başlık altında toplanmıştır: 1)Protez kaide maddelerinin ısı iletkenlikleri karşılaştırılmış ve ısı iletkenlik özelliklerinin protezde ısı karşısında oluşan boyutsal değişikliğe etkisi araştırılmıştır. 2)Bunun yanında boyutsal değişiklik çalışmalarında yeni bir boyutsal değişiklik ölçüm yöntemi ve 3)örnek şekli kullanılmış ve bu yöntemlerin doğruluğu araştırılmıştır. 4)Araştırmada, tesviye ve polisaj işleminde oluşan lokal sıcaklık artışları ve bunun sonucu olarak 5) tesviye ve polisaj işlemlerinin boyutsal değişiklik üzerine etkileri araştırılmıştır. 6)Genel sıcaklık artışlarında, protezlerde oluşan boyutsal değişikliklerin yanında bu değişikliklerin ne zaman başladığı ve ne miktarda meydana geldiği ve 7)protez kaidesinin kalınlıklarının etkileri araştırılmıştır. 8)Bunun yanında maddenin su emilim özelliğinin boyutsal 266stabilfte üzerine etkisi araştırılmıştır, ölçümlerin istatistiksel analizleri GraphPad Prizma V.3 paket programı ile yapılmıştır. Araştırmamızın sonuçlarına göre kullanılan farklı akrilik protez kaide maddelerinin ısı diffiüzyonları farklı, ısı iletkenlikleri birbirinin aynı bulunmuştur. Isı difüzyonundaki bu farklılık farklılık lokal sıcaklık uygulamalarında oluşan boyutsal değişikliğe yansımıştır. Genel sıcaklık uygulamalarında ise polimerin ısı iletkenliği ve ısı difüzyonunun oluşan boyutsal değişikliğe etkisinin olmadığı bulunmuştur. Protezin tesviyesi polisajı ideal koşullarda yapılsa bile bir boyutsal değişikliğin meydana geldiği bulunmuştur. Fakat bu oluşan boyutsal değişiklikler çok az miktardadır ve göz ardı edilebilir. Boyutsal değişiklik çalışmalarında düz at nalı veya anatomik şekle sahip örneklerde oluşan boyutsal değişiklikler aynı bulunmuştur. Yeni tanımlanan ölçüm yönteminin "dijital fotoğrafların ölçümü yöntemi" hassasiyeti, akrilikte oluşan boyutsal değişikliklerin belirlenmesi için yeterli değildir. Bu yöntemin daha geliştirilmesi gerekmektedir. Akrilik maddesi camsı geçiş sıcaklıklarının çok altında sıcaklıklarda boyutsal değişikliğe uğramaya başladığı saptanmıştır. Fakat sıcak akrillerin 100°C, soğuk akrillerin 90°C sıcaklıkta tamamen deforme oldukları saptanmıştır. İnce protezlerin kalın protezlere nazaran daha fazla boyutsal değişikliğe uğradığı bulunmuştur. Sıcaklık uygulamaları ve su emilimi sonucu oluşan boyutsal değişiklikler incelendiğinde ideal protez kalınlığı 3mm olarak saptanmıştır. Kullanılan protez kaide maddelerin su emilimi sonucu oluşan boyutsal değişiklikleri incelendiğinde sadece Vertex sıcak akrilde oluşan boyutsal değişikliğin istatistiksel olarak anlamlı olduğu saptanmıştır. Kuru ortamda bırakılan protezlerde, su içinde 267bırakılan protezlere nazaran daha fazla boyutsal değişikliğin meydana geldiği saptanmıştır. Bu doktora çalışmasının bulgularına göre aşağıdaki sonuçlar söylenebilmektedir: 1. Akrilik protez kaide maddelerinin ısı difüzyon özellikleri lokal sıcaklık uygulamalarında oluşan boyutsal değişiklikliği etkilemektedir. 2. Protezin tesviyesi sırasında akrilik yüzeyde lokal sıcaklık artışı oluşmaktadır. 3. Düz at nalı örnek şekli alt protezi temsil etmektedir. 4. Dijital fotoğrafların ölçüm yöntemi ideal bir yöntem değildir. 5. İdeal protez kalınlığı 3mm'dir. 6. Tesviye-polisaj işleminde oluşan boyutsal değişiklik göz ardı edilebilir. 7. Soğuk akriller 90°C, sıcak akriller 100°C sıcaklıkta tamamen deforme olmaktadır. 8. Protezler ağıza takılana kadar su içerisinde bekletilmelidir. THE EFFECT OF GENERAL AND LOCALIZED HEAT APPLICATION ON DIMENSIONAL STABILITY OF ACRYLIC DENTURE BASE MATERİALS Even if the ideal conditions have been made during the laboratory procedure of acrylic material, there will be expansions and contractions dependent upon its nature during or after the polimerisation. Changes of acrylic resin affect the whole structure of the prosthesis and cause dimensional changes. One of the reason which causes dimensional changes, may be the general and localized heat. The localized temperature rise which affects prosthesis, occurs during the grinding and polishing. The general temperature rise on the whole surface of the prosthesis occurs as a result of the hot food and drink or subjected to the hot liquid. In this study, the dimensional changes of lower complete dentures and their reasons have been evaluated when heated. This study have been converted in eight topics: 1)The heat conductivity of acrylic denture base materials have been compared and how their heat conductivity properties affect the dimensional changes on dentures which occur during heating are tested. 2) Moreover a new measurement method for the determination of dimensional accuracy and stability and 3)a new sample shape have been used in this study. Also the accuracy and reliability of the new methods have been investigated. 4) In the research, the localized heat, generated during grinding and polishing and 5) its affect on the dimensional stability of the lower complete dentures 270have been investigated. 6) In the other hand, the effect of general heat increase on the dimensional stability of acrylic dentures, 7) at which temperature and what quantity these dimensional changes occur, the effect of denture thickness on the dimensional stability have been investigated. 8)However the efect of water sorption on dimensional stability of polymeric materials have been investigated. The statistical analysis of the measurements have been made using GraphPad V.3 programme. The study showed that, the heat conductivity of the used denture base materials are the same but the heat diffusion of the used denture base materials are different from each other and these differences affect the dimensional stability of the dentures in local heat applications. But it was found that the heat conductivity and heat diffusion properties of polimeric denture base materials do not affect the dimensional stability in general heating. Even if the ideal conditions are provided, a dimensional change is determined during grinding and polishing the dentures. But this change is so small and can be undervalued. More dimensional chages occur in thin dentures than in thick ones. When determining the dimensional change of dentures, the use of flat horse shoe sample shape reflects the changes occuring at the anatomic shaped samples. The accuracy of defined new method of determining dimensional changes by computer controled measuring of digital images have not enough accuracy in determination of dimensional changes of acrylic dentures. This new method must be developed. Dimensional changes in acylic resins occur at the lower temperatures than their glass transition temperature. It has been found that, 271exact deformation of heat cure acrylics occur at 100°C and cold cure acrylics at 90°C. The dimensional changes due to heat application and water sorbtion have been examined and found that, only the dimensional change at water sorption of Vertex heat cure acrylic is statistically significant, more dimensional changes occur when dentures are left on dry condition and the ideal denture thickness is 3mm. As a result of this study, these conclusions can be determined: 1. The heat diffusion of acrylic denture base materials affects the dimensional stability at local heat application. 2. During grinding a heat formation occur at the surface of acrylic dentures. 3. Flat horse shoe specimen shape represents anatomic lower denture. 4. The measurement of digital images is not an ideal metod on determination of dimensional change. 5. The ideal denture thickness is 3mm. 6. After grinding and polishing the dimensional change can be undervalue. 7. Exact deformation of heat cure acrylics occur at 100°C and cold cure acrylics at 90°C. 8. Dentures must be left in water until insertion to mouth. Key Words: Heat conductivity, dimensional stability, acrylic resin, denture base 272
