Tarsal koalisyon, sıklıkla çocukluk yaş grubunda özellikle adölesan çağda bulgu vermeye başlayan, ayakta ağrı ve hareket kısıtlılığı ile bulgu veren, iki yada daha fazla tarsal kemik arasında anormal bağlantı ile karakterize rijit düztabanlığın sebeblerinden birisidir. Çalışmamızda ağrılı tarsal koalisyonu bulunan hastaların klinik seyirlerini ortaya koyulması amaçlandı. Çalışmaya tarsal koalisyon tanısı almış, konservatif ve cerrahi tedavileri 2003-2013 yılları arasında yapılan hastalar alındı. Konservatif tedavi edilen hastalar (G1;14 hasta) ve cerrahi uygulanmış hastalar (G2;16 hasta) olarak iki grupa ayrıldı. Çalışmaya alınan 30 hastanın (5K/25E) yaş ortalaması 26,5±10,6 olarak tespit edildi. Çalışmaya dahil edilen hastalarda takip süresi G1'de ortalama 70,7±28,4 ay olduğu, G2'de cerrahi tedavi öncesi ortalama 29,1±15,8 ay takip edildiği ve cerrahi sonrası ortalama takip süresinin 49,6 ±39,2 ay olduğu tespit edildi. Hastaların ayak şikayetlerinin başlangıç döneminde ve ilk aldıkları konservatif tedavi sonrası ağrı seviyesi VAS skalası ile değerlendirildi. G1'de şikayetlerinin başlangıç evresinde ortalama VAS skoru 4,9±1,9 ve ilk aldıkları konservatif tedaviler sonrası ortalama VAS skoru 2,8±1,0 iken G2'deki hastalarda bu değerler sırası ile 7,7±1,3 ve 7,1±0,8 olarak tespit edildi. Her iki grup arasındaki VAS ortalamaları arasındaki fark istatistiksel olarak anlamlı idi (p<0,05). Çalışmamızda hastalara son kontrollerinde ağrı düzeyleri VAS skalasına göre hareketle, istirahette ve gece olmak üzere üç faklı zaman diliminde sorgulandı. G1'de hereket esnasında ortalama VAS skoru 2,3±2,4 iken, dinleme esnasında 0,6±0,9 ve gece ortalama VAS skoru benzer şekilde 0,6±1,7 olarak tespit edildi. Cerrahi tedavi uygunan grubta son kontrolde hereket esnasında ortalama VAS skoru 3,1±2,7 iken dinleme esnasında 0,6±1,7 ve gece ortalama VAS skoru 0,5±1,1 olarak tespit edildi. Gruplara göre son kontroldeki hareketli, dinlenme esnası ve gece oluşan VAS skorları istatistiksel olarak anlamlı farklılık göstermemektedir (p>0,05). Buna göre G1'deki hastaların hastalığın başlangıç evresinde ağrı seviyelerinin daha düşük olduğu aldıkları konservatif tedavilerle tolere edilebilir ağrı seviyesi elde edildiği kanaatine varıldı. G2'de ise hastaların başlangıçtan itibaren yüksek ağrı seviyelerine sahip oldukları ve bunun ancak cerrahi tedavi sonrası tolere edilebilir seviyelere gerilediği izlendi. VAS skoru başlangıçta 5,5 ve üzeri olan hastaların ağrı seviyerlerinin gerilemediği cerrahi tedavi gereksinimin erken dönemde ortaya çıktığı çıkarımı yapıldı. Tarsal coalition, characterzied by abnormal connection between two or more tarsal bones and causing pain and loss of movement in the foot, is mostly symptomatic during the pediatric age –especially during adolescence-; and it is one of the main reasons of rigid flat foot. The aim of our study was to investigate the clinical course of the patients diagnosed with painful tarsal coalition. Patients diagnosed with tarsal coalition and treated conservatively and surgicallly between 2003 to 2013 were enrolled in the study. They were grouped into two groups as the ones who were treated conservatiively (G1i 14 patients) and the ones who were treated surgically (G2; 16 patients). 30 patients (5 females;25 males) with a mean age of 26,5±10,6 years were enrolled in the study. The mean follow up period o was 70,7±28,4 months for G1 and 29,1±15,8 months for G2 pre-operatively; and 49,6 ±39,2 months post-operatively. VAS score was used to evaluate the foot complaints of patients at the very beginning and after the first conservative treatment they received. In G1 mean VAS score at the very beginning was 4,9±1,9 and after the first conservative treatment was 2,8±1,0; whereas in G2 mean VAS score at the very beginning was 7,7±1,3 and after the first conservative treatment was 7,1±0,8. The difference of the average VAS score between the two goups was statistically significant (p<0.05). The level of patients' pain was questioned in their last follow-up, in three time intervals, as in motion, during resting and at night. In G1, VAS score during motion was 2,3±2,4, during resting and night was 0,6±1,7. Patients treated surgically were reported during the last follow-up visit to have a mean VAS score of 3,1±2, during motion, 0,6±1,7 during resting and 0,5±1,1 during night. VAS scores during motion, resting and night showed no statistically significant difference (p>0.05). As a result of that, it was concluded that patients in G1 had a lower degree of pain at the beginning and acquired a tolerable level of pain as a result of the conservative treatments they received. In G2, it was reported that pain subsided to tolerable levels. It was noted that patients with a VAS score of 5.5 at the beginning need surgery in the early period because the level of pain was not regressing. Keywords: Tarsal coalition, VAS, Clinical course
