Şizofreni; nedeni henüz kesin olarak bilinmeyen bir nöropsikiyatrik bozukluktur. Şizofreninin
heterojen bir bozukluk olduğu ve birden fazla etiyolojik süreç içerdiği düşünülmektedir.
Çalışmalar güçlü bir genetik komponenti işaret etmekle birlikte, epidemiyolojik verilerin çoğu
bazı durumlarda şizofreninin enfeksiyon hastalıkları ile ilişkili olabileceğini göstermektedir.
Yapılan çalışmalarda birçok mikroorganizma (Herpes Simplex Virus Tip 1 ve Tip 2 (HSV 1-
2), Cytomegalovirus (CMV), Influenza virus, Klamidya spp., Toxoplasma Gondii gibi)
şizofreni için risk faktörü olarak gösterilmiştir. Şizofreni ile ilişkilendirilen
mikroorganizmaların ortak özellikleri merkezi sinir sistemine (MSS) yerleşme yeteneğine sahip
olmalarıdır. Çalışmamızda araştırdığımız bu çift sarmallı DNA viruslarından bazıları (HSV-1,
HSV-2, CMV) merkezi sinir sistemini enfekte ettikten sonra, yıllarca konak hücrede latent
kalabilirler ve periyodik olarak yeniden aktif hale gelebilirler. Şizofreni hastalığını adli bilimler
içerisinde en önemli kılan konu psikiyatrik bozukluklar içinde saldırgan davranışla en ilişkili
grup olarak gösterilmesidir. Şizofreni hastalarının genel populasyona oranla daha saldırgan
olduğuna dair çalışmalar bulunmaktadır. Şizofreni hastalarındaki şiddet potansiyeli toplumu
korkutmakta ve psikiyatrik bozuklukları olan kişilerin toplumda kabul görmesini
engellemektedir. Şizofreni aynı zamanda tüm dünyada büyük oranda kişisel ve ekonomik
sorunlara yol açan en önemli sosyolojik sorunlardan biridir. Adli bilimlerin içerisinde
özelleşmiş olan adli psikiyatri açısından şizofreni hastalarının ceza sorumluluğu konusu önemli
bir yer teşkil etmektedir. Bunun yanı sıra psikiyatri; suç işlememiş hastaların tedavilerinde
semptomların hafifletilmesine odaklanırken, tedavideki amaç, psikopatolojinin remisyonunu
sağlamaya çalışmak, gelecekteki potansiyel şiddet davranışlarını azaltmak ya da elimine
etmektir. Ayrıca yeni suçları azaltarak toplumu korumak, bu bireylerin sağlıklı gelişimlerine ve
kişilerarası ilişkilerde sorumluluk almalarına yardımcı olmaktır. Hem ruh sağlığı hem de adli
sistem arasındaki ilişkinin başarılı olabilmesi için iletişimin açık, sık ve devamlı olması

gerekmektedir. Birlikte, hastanın en etkili tedavi programı düzenlenmeli ve geliştirilmelidir.
Şizofreni hastaları ve sağlıklı kontrol grupları arasında enfeksiyöz hipotez (nörogelişimsel
varsayım) konusunda biyolojik psikiyatri alanında çok çeşitli çalışmalar bulunmaktadır. Buna
ek olarak suç işleyen ve işlemeyen grup arasında ise viral ajanların etkisiyle ilgili araştırmaya
rastlanmamıştır. Dolayısıyla araştırmamızda şizofreni hastaları adli öyküsü olan grup (suç
işleyen) ve adli öyküsü olmayan (suç işlemeyen) grup olarak sınıflandırılmıştır. Bu grupların
sosyodemografik özellikleri incelenmiştir. Hastalardan alınan serumlardan da serolojik
yöntemle bazı viral etkenler araştırılarak her iki grup arasında karşılaştırma yapılmıştır.
Çalışmamız bu yönüyle hem öncü bir çalışmadır hem de farklı mikroorganizmalarla yapılacak
çalışmalara ve genetik çalışmalara yol gösterici olacaktır.

Schizophrenia is a neuropsychiatric disorder whose cause is yet to be discovered. It has been
thought that it is a heterogeneous disorder and it does contain multiple etiological processes.
Studies point out a strong genetic component, whereas, most of the epidemiological data reveals
the presence of a schizophrenia's relation to infectious diseases in some cases. In the studies
conducted, multiple microorganisms, such as Herpes simplex 1-2 (HSV 1-2), Cytomegalovirus
(CMV), Influenza, Chlamydia spp., Toxoplasmosis gondii have been shown as risk factors. The
common feature of the microorganisms that have been associated with schizophrenia has a
capability of settling in the Central Nervous System (CNS). Some of the double-strand DNA
viruses (HSV 1, HSV 2, CMV) we studied might infect the central nervous system and then
become latent within the host cell for many years and reactivate periodically. Schizophrenia
plays an important part in forensic sciences due to the fact that it has the strongest relation to
aggressive behavioral patterns within psychiatric disorders. There are studies, revealing that
schizophrenic patients are more aggressive compared to the rest of the population in general.
Possible violent acts schizophrenic patients exhibit scare the society and so, this prevents the
acceptance of people with psychiatric disorders into the society. Schizophrenia is, also, one of
the sociological problems, causing personal and economical disadvantages in the entire world.
The penal responsibility of schizophrenic patients is a significant issue in forensic psychiatry,
special branch within forensic sciences. In addition to this, psychiatry focuses on lightening the
symptoms of patients, who have not committed any criminal acts before, during their course of
treatment. The aim of the therapy is to work on the remission of the psychopathology, to
decrease the number of future violent acts or to eliminate them, to protect the society in its
entirety by decreasing the number of new violent acts, and to help these individuals in the areas
of healthy self-improvement and responsibility-undertaking regarding interpersonal relations.

In order for a relation between mental health and judicial system to be successful; open,
frequent, and a continuing communication should be obtained. The patient's treatment schedule
should be arranged and improved, if necessary in correlation with one another.
There are variable studies in the area of biological psychiatry, regarding infectious hypothesis
(neurodevelopmental assumption) between schizophrenic patients and healthy control groups.
In addition to this, it is important to highlight that no research has been conducted regarding the
comparison of the affect of the viral agents in the groups with and without criminal
backgrounds. Hence, in our study, schizophrenic patients have been divided into offender's
group (the group with criminal acts) and non-offender's group (the group without criminal acts).
Socio-demographic features of these groups have been analyzed. Patients' blood samples have
been serologically studied for comparison between two groups regarding some viral agents.
Our study is, from this aspect, both a pioneer and a leading study for future genetic studies and
other studies that include variable microorganisms. |
