Dikkat Eksikliği Hiperaktivite Bozukluğu (DEHB), çocukluk çağında başlayıp erişkinlikte de devam eden nöropsikiyatrik bozukluktur. Depresyon en yaygın görülen duygu durum bozukluğudur. Bu iki bozukluk erişkinlerde sık görülen psikiyatrik bozukluklardan olmalarına rağmen tedavisinde kullanılan ilaçların genotoksisitesi hakkındaki bilgiler kısıtlıdır. Bu çalışmada DEHB ve Depresyon tedavisinde kullanılan ilaçların genotoksisitesinin değerlendirilmesi amaçlanmıştır. Çalışmaya yeni DEHB ve/veya Depresyon tanısı alan 18 ile 60 yaş aralığındaki 101 hasta dâhil edilmiştir. Tüm katılımcılar Eksen-I Bozuklukları için Yapılandırılmış Klinik Görüşme (SCID-I) kullanılarak psikiyatrik bozukluklar için taranmıştır. Komorbid psikiyatrik bozukluğu olan tıbbi hastalık öyküsü olan ya da psikotrop ilaç kullanımı olan hastalar çalışma dışı bırakılmıştır. Hastalar DEHB, Depresyon ve DEHB+Depresyon olmak üzere 3 gruba ayrılmıştır. Hastalara gruplarına göre tedavi öncesi ve sonrası tedavi etkinliğini değerlendirme amacıyla ölçekler uygulanmıştır. Hastalara başlanan ilaçlar ve dozları natüralistik olarak belirlenmiştir. Tüm bireylerden tedaviden önce ve tedaviden 2 ay sonra 5 ml venöz kan örnekleri alınıp, lökositleri ayrıştırılarak analiz gününe kadar -80°C de saklanmıştır. Lökositlerde DNA hasarı parametrelerini ölçmede comet analizi yöntemi kullanılmıştır. Çalışma sonucunda MPH ve seçici serotonin geri alım inhibitörleri (SSGİ) tedavisinin genotoksik etkisinin olduğu saptanmıştır. Fluoksetin tedavisi alan hastalarda genotoksisite saptanmamıştır. MPH dozunun (28mg/gün ve üstü) artması ile genotoksik etki kaybolmuştur. MPH tedavisi alanlarda sigara kullanımının genotoksisiteyi arttırdığı saptanmıştır. Bu çalışmada lökositlerde tespit edilen DNA hasarı bize MPH ve SSGİ grubu ilaçların daha dikkatli ve terapötik dozlarda kullanılması gerektiğini göstermektedir.
Attention Deficit Hyperactivity Disorder (ADHD) is a neuropsychiatric disorders starting in childhood also ongoing in adulthood. Depression is the most common mood disorder. Despite these disorders are common psychiatric disorders in adults; information on the genotoxicity of the drugs used in the treatment of these disorders is limited. This study aimed to evaluate the genotoxicity of drugs used to treat ADHD and depression. 101 patients between 18-60 years of age newly having a diagnosis of depression and/or ADHD were included into the study. All participants were screened for psychiatric disorders by using Structured Clinical Interview for Axis I Disorders (SCID-I). Patients with comorbid medical illness, psychiatric disorders or using psychotropic medication were excluded from the study. Patients were divided into 3 groups such as those with ADHD, depression and ADHD+Depression. To evaluate the efficiency of the teratment, patients were given some scales before and after treatment. Drugs and doses applied to the patients were determined as naturalistic. From all subjects before treatment and after 2 months of treatment 5 mL venous blood samples were received, leukocytes were separated and stored at -80°C until the day of analysis. Comet analysis is used to measure DNA damage parameters in leukocytes. The results showed that significant genotoxicity was determined in the DNA of patients who took MPH and selective serotonin reuptake inhibitors (SSRIs) after the treatment compared to those before treatment according to the comet analysis results. However, there was no significant difference before and after treatment in those who took fluoxetine among SSRIs in terms of genotoxicity. The DNA of those using lower doses than 28 mg/day MPH treatment was significantly damaged, while those taking higher doses than 28 mg/day MPH showed no significant difference. Those who are smoking besides taking MPH treatment were observed to have more DNA damage compared to nonsmokers. As a result of this study, the detection of DNA damage in leukocytes suggests that MPH and SSRIs should be used in therapeutic doses and, should be more attentive while using this medication.
