‹leri evre küçük hücreli d›fl› akci¤er kanseri (KHDAK) tedavisinde gemsitabin-sisplatin kemoterapisinin etkinli¤i araflt›r›ld›. 1999- 2002 tarihleri aras›nda klini¤imizde izlenen ve gemsitabin-sisplatin protokolu uygulanan 48 hastan›n verileri retrospektif olarak gözden geçirildi. Dosya kay›tlar› histolojik tip, evre, kemoterapi kür say›s›, kemoterapi ile elde edilen yan›t, toksisite ve genel sa¤kal›m aç›s›ndan incelendi. Toplam 48 non-rezektabl KHDAK'li hastan›n de¤erlendirilmeye al›nd›¤› çal›flmada 46's› erkek 2'si kad›n olup, yafllar› 39-74 aras›ndayd› (yafl ortalamas›: 64). Hastalar›n 26's› (%54) evre III B, 22'si (% 46) evre IV idi. Histolojik alt tip da¤›l›m› flöyleydi: 20 hasta (% 41) skuamöz hücreli, 9 hasta (% 19) adenokarsinom, 1 hasta (%2) büyük hücreli, 18 hasta (% 38) küçük hücreli d›fl›. Hastalar›n tümüne gemsitabin-sisplatin kemoterapi rejimi ilk s›ra tedavi olarak verildi ve 234 kür uyguland›. 2 kür sonras› yap›lan de¤erlendirmede yan›t elde edilen olgularda ayn› rejimle tedaviye devam edilerek tedavi 6 küre tamamland›. ‹lk s›ra gemsitabin-sisplatin protokolu ile 1 hastada tam yan›t (%2.09), 27 hastada k›smi yan›t (%56.25) elde edildi. Median sa¤kal›m 13.2 ay olarak bulundu. Evre IIIB KHDAK'li olgularda median sa¤kal›m ve 1 y›ll›k sa¤kal›m s›ras›yla 13.4 ay ve % 51 bulundu. Evre IV KHDAK'li olgularda median sa¤kal›m ve 1 y›ll›k sa¤kal›m s›ras›yla 12.3 ay ve % 36 bulundu. Grade 4 anemi % 2.3 , Grade 4 nötropeni % 9.4, Grade 4 trombositopeni ise % 10.25 oran›nda izlendi. Sonuç olarak evre IIIB ve IV KHDAK'li hastalarda gemsitabin- sisplatin rejiminin etkili ve tolere edilebilirli¤i yüksek bir kombinasyon oldu¤u tesbit edildi. | |||
The effectiveness of gemcitabine-cisplatin chemotherapy in the treatment of advanced stage non-small cell lung cancer was evaluated. Data from 48 patients followed in our clinic and treated with gemcitabine-cisplatin protocol between 1999-2002 reviewed respectively. File records were evaluated according to histological type, grade, number of chemotherapy cycle, response to chemotherapy, toxicity and overall survival. 46 of 48 non-resectable NSCLC patients were male, 2 of them were female and their ages were ranged between 39 to 74 (average age were 64). 26 patients (54%) were stage IIIB and 22 of them (46%) were stage IV. The distribution of histological subtype were as follows: 20 patients with squamous cell type (41%), 9 patients with adenocarcinoma (19%), 1 patient with large-cell type (2%), 18 patients with non-small cell type (38%). All of the patients received gebcitabinecisplatin chemotherapy as a first line therapy and continued for total number of 234 cycles. Patients with favorable response after 2 cycles continued with the same regimen and completed to a total of 6 cycles. Through the gemcitabine-cisplatin protocol, 1 patient experienced a complete response (2.09%) while 27 patients showed partial response (56.25%). The median survival was 13.2 months. The median survival and one year survival rate for stage IIIB patients were 13.4 months and 51% respectively. For NSCLC satege IV patients the median survival and one year survival rate were 12.3 months and 36% respectively. Grade 4 anemia was observed in 2.3% of the cases while grade 4 neutropenia and grade 4 thrombocytopenia were seen in 9.4% and 10.25% of the patients, respectively. As a result, this study showed that gemcitabine-cisplatin regimen is an effective combination with a favorable tolerability and toxicity profile. |
