Kontinens, kişinin defekasyon ihtiyacını kabul edilebilir bir süre için erteleyebilme yetisidir. Kontinensin sağlanmasında sfinkterler, bunların uyarımını sağlayan nöral yollar, mental durum gibi faktörlerin herbirinin sağlıklı olarak çalışması gereklidir. Bunlardan birinde meydana çıkan işlevselsizlik sonrasında barsak içeriğinin planlanabilir olarak boşaltılamaması, ertelenememesi ortaya çıkar ve ?anal inkontinens? olarak adlandırılır.Anal inkontinens çok sayıda etyolojik faktörle ortaya çıkabilir, kişilerin hayat konforunu, ruhsal dengesini, sosyal ilişkilerini ve iş güçlerini olumsuz yönde etkileyebilir.Her yaş grubunu etkileyebilen anal inkontinens %1,4-%12,7 arasında değişen prevalansa sahiptir. Değişik tedavi modelleri ile tedavi edilebilmektedir.?Anal İnkontinensli Hastalarda Sfinkteroplasti Sonuçlarınının Değerlendirilmesi? çalışması için Şubat 1998-Kasım 2008 tarihleri arasında İstanbul Tıp Fakültesi Genel Cerrahi Anabilim Dalı B Servisi'nde inkontinens şikayetleri ile başvuran ve yapılan tetkilerinde sfinkter hasarı bulunan 52 hastaya uygulanan sfinkteroplasti sonuçlarının değerlendirilmesi planlanmıştır.Sonuç olarak; 52 hastanın 8'inde (%15,4) yara infeksiyonu gelişti ve bunların 4'ü stomalı hastalardı. Stomalı ve stomasız hastalarda yara infeksiyonu açısından istatiksel fark saptanmadı (p>0,05) Fischer's exact test.15 hastanın ameliyat öncesi stoması mevcuttu ve bunlar Wexner skorlamasına dahil edilmedi. 37 hastanın ameliyat öncesi ortalama Wexner skoru 15,44±5,32 idi. Sfinkteroplasti (SP) sonrası 3 ve 6. ay ortalama Wexner skorları 5,67±5,270 ve 12. ayda 5,50±5,38 oldu. Skordaki gerileme istatiksel olarak anlamlıydı (Wilcoxon signed ranks test).Her iki SP tekniği karşılaştırıldığında; Overlap SP yapılan 20 ve uç uca SP yapılan 17 hastanın ameliyat sonrasında Wexner skorlarında anlamlı azalmalar saptanmakla birlikte iki tekniğin birbirine başarı üstünlüğü bulunmamıştır (Wilcoxon signed ranks test).
Continence is the ability of a person to postpone the need of defecation for an acceptable period of time. In order to maintain continence; all of the factors such as the sphincters, the neural routes that innervate them, and the mental state must work healthily. After a disfunction in any of them, the inability to empty the colonic contents in a planned manner, the inability to postpone occurs, and it is called ?anal incontinence.?Anal incontinence can appear with multiple etiologic factors, and can affect the life comfort, the mental equilibrium, the social affairs and the working energy of people in a negative manner.The anal continence that can affect all the age groups has a prevelance ranging from 1.4% to 12.7%. It can be treated with different treatment models.For the ?Evaluation of the Results of Sphincteroplasty for Patients with Anal Incontinence? study, it is planned to evaluate the results of sphincteroplasty applied to 52 patients who presented with the complaints of incontinence, and whose examination revealed sphincter injury at the Istanbul Faculty of Medicine, General Surgery Department, B Service between the dates of February 1998 to November 2008.As a result; in 8 of the 52 patients (15.4%), surgical site infection occurred, and 4 of them occurred in patients with stoma. No statistical difference was detected in the aspects of surgical site infections in patients with or without stoma. (p>0.05) Fischer?s exact test.15 of the patients had stoma before the operation, and they were not included in the Wexner score. The average Wexner score of 37 patients before the operation was 15.44±5.32. The average Wexner scores 3 and 6 months after the sphincteroplasty (SP) was 5.67±5.270, and it was 5.50±5.38 after 12 months. The regression in the score was statistically meaningful. (Wilcoxon signed ranks test)When both SP techniques were compared; after the operation of 20 patients with SP overlap applied, and of 17 patients with end-to-end SP applied, meaningful decreases in the Wexner scores were determined besides the finding that both techniques did not have superiorities over each other.
