ÖZET Baş boyun kanserlerinin tedavisinde kullanılan antineoplastik ilaçlar tümör ve normal dokular arasında seçici olmadıklarından dolayı bazı normal dokuları da hasara uğratırlar. Oral mukoza yüksek mitotik indekse sahip olması nedeni ile, kemoterapi toksisitenin görüldüğü en önemli bölgelerden olup, tükürük ve tükürük bezleri de kemoterapinin toksik etkilerinden şiddetli olarak etkilenmektedir. Antineoplastik ilaçlarla tedavi gören hastalarda, tükürük bezi hipofonksiyonlanna bağlı olarak gelişen kuru ağız semptomları, tad alma bozukluğu, yutma güçlüğü, tükürük komponentierinin antimikrobiyal potansiyelinin azalması ile kandida enfeksiyonuna açık hale gelmesi gibi yan etkiler, hastanın yaşam kalitesini dramatik bir şekilde düşürmektedir. Çalışmamızda, baş boyun bölgesi kanserlerinin tedavisinde yaygın olarak kullanılan antineoplastik ilaçlardan olan ADR'in, sisplatinin, MTX'in ve 5-FU'in mikst bir tükürük bezi olan sıçan submandibuler tükürük bezleri üzerine olan yan etkileri bu fikirden yola çıkılarak değerlendirilmiştir. Bu etkiler değerlendirilirken morfolojik ve işlevsel parametreler esas alınmıştır. Bu değerlendirmelere göre sıçan submandibuler tükürük bezlerinde en fazla toksik etkiyi 5-FU'in gösterdiği, bu sırayı ADR ve sisplatinin izlediği sonucuna ulaşılmıştır. MTX ise en az toksik etkiyi gösteren antineoplastik ilaçtır. 60
SUMMARY The antineoplastic agents used for the treatment of head and neck cancer cause the damages to the healthy tissues because of the fact that the agents have also side effects on the healthy tissues. Owing to its high level of mitotic index, the oral mucosa is an important area where the presence of chemotherapy toxicity has been often encountered. In addition, saliva and salivary glands are also adversely affected by the chemotherapy toxicity. In those patients treated with antineoplastic agents, side effects such as dry mouth commonly associated with hypofunction of the salivary glands, taste disorder (hypogeusia), swallowing problems, increase in susceptibility to Candida infections due to the decrease in its antimicrobial potential, have worsened the patient quality of life. In our study, the side effects of antineoplastic agents such as adriamycin, cisplatin, methotrexate, and 5-fluorouracil, which are commonly used for the treatment of head and neck cancer, on submandibular salivary glands of rat, which has the mixed salivary glands, have been evaluated from this point of view. Evaluation of these effects has been based on the morphologic and the functional parameters. In conclusion, the most toxic effects on submandibular salivary glands of rat are found to be caused by the 5-fluorouracil, the second one is adriamycin, and the third one is cisplatin. Methotrexate is the antineoplastic agent having the least toxic effects. 61
