Amaç: Çalışmamızda, böbrek kitlesi olan hastalarda kitlenin histopatolojik tip ve Fuhrman Derecelendirme Sistemi'nin (FDS), biyopsi/operasyon öncesinde Manyetik Rezonans Görüntüleme (MRG)'de kullanacağımız sekanslar ile saptayabilmeyi amaçladık. Elde edilen bilgiler ışığında da kitle karakterizasyonu için önemli olan sekansları belirleyip elimizde MRG gibi radyasyon güvenliği olan kesitsel inceleme tekniğinin, hasta konforunu rahatsız eden süresini kısaltabilmeyi ve histopatolojik tip ile ilişkili tedavi planının şekillenmesine biyopsi/operasyon öncesinde katkı sağlamayı hedefledik. Gereç ve Yöntem: Çalışmamız retrospektif olup 2010-2019 yılları arasında hastanemiz SBÜ İzmir Bozyaka EAH'de cerrahi tedaviye gitmiş, histopatolojik tip- derecesi elde olunmuş 83 böbrek tümörlü hastanın, hastanemizde preoperatif 1,5 Tesla MR cihazları ile elde olunmuş dinamik üst batın MRG'lerinin değerlendirilmesini kapsamaktadır. Çalışmada değerlendirilen parametreler; hastaların yaş ve cinsiyet dağılımı; kitlenin boyutu; kitlenin aksiyel planda en geniş çapında ve o kesitte çap içinde nekroz/kist/kalsifikasyon içermeyen, kontrast madde tutan solid komponentine ROİ koyularak o odakta T1 ağırlıklı görüntüleme (T1AG) ve T2 ağırlıklı görüntülemede (T2AG) sinyal intensite (si) özellikleri; intraselüler yağ varlığı; kontrast madde tutma ve kontrast maddeden yıkanma özellikleridir. Bu parametreler ile renal hücreli karsinomlar(RHK) arasında ve berrak hücreli RHK'lerin (BHRHK) histopatolojik olarak FDS'ye göre düşük ve yüksek dereceli subtiplerinde ayrım yapabilmeyi hedefledik. Bulgular: Çalışmamızdaki 83 hastanın 33'ü kadın (%39,8), 50'si (%60,2) erkekti. Hasta yaş ortalaması 57,54 ±12,63'tü. Çalışmamıza 4 anjiomyolipom (AML), 4 onkositom olmak üzere 8 benign lezyon; 42 BHRHK, 19 papiller RHK (PRHK) ve 14 kromofob RHK(KRHK) olmak üzere 75 malign lezyon incelemeye dahil edilmiştir. Benign ve malign lezyonlar arasında hasta yaş ortalaması (p=0,14), kitlenin en uzun çap ortalaması (p=0,23), lezyonun endo-ekzofitik yerleşimi açısından (p=1,00), dominant komponenti kistik/nekrotik-solid olma özelliği açısından (p=0,56) istatiksel olarak değerlendirildiğinde anlamlı faklılık saptanmadı. BHRHK ve nonberrak hücreli RHK'ler (NBHRHK) arasında hasta yaş ortalaması (p=0,57), kitlenin en uzun çap ortalaması (p=0,77), lezyonun endo-ekzofitik yerleşimi açısından (p=0,62), dominant komponenti kistik/nekrotik-solid olma özelliği açısından (p=0,84) istatiksel olarak değerlendirildiğinde anlamlı faklılık saptanmadı. BHRHK, PRHK ve KRHK'ler arasında hasta yaş ortalaması (p=0,24), kitlenin en uzun çap ortalaması (p=0,86), dominant komponenti kistik/nekrotik-solid olma özelliği açısından (p=0,19) istatiksel olarak değerlendirildiğinde anlamlı faklılık saptanmadı. Düşük dereceli berrak hücreli RHK (DDBHRHK) ve yüksek dereceli berrak hücreli RHK'ler (YDBHRHK) arasında hasta yaş ortalaması (p=0,70), lezyonun endo-ekzofitik yerleşimi açısından (p=0,47), dominant komponenti kistik/nekrotik-solid olma özelliği açısından (p=0,70) istatiksel olarak değerlendirildiğinde anlamlı faklılık saptanmadı. Sadece kitlenin en uzun çap ortalamaları arasında istatiksel olarak anlamlılık (p=0,01) saptandı. BHRHK ve NBHRHK'ler arasında, tümörün en geniş çapında ve o çapta konulan ROİ ile saptanan odakta; T2AG'de yağ baskısız serilerde si özelliği, T2SİO, T1SİO, TDSİO, Wiİ1-2-3 ve Woİ1 değerleri istatiksel olarak değerlendirildiğinde anlamlı olarak (p<0,05) saptandı. T1AG'de si değeri geniş çapta anlamlı (p=0,04) çıkıp odakta (p=0,36) çıkmadı ve Woİ2 değeri odakta anlamlı (p<0,01) çıkıp geniş çapta (p=0,17) çıkmadı. BHRHK, PRHK ve KRHK'ler arasında en geniş çap ve odakta; T2SİO, T1SİO, TDSİO, Wiİ1-2-3 ve Woİ1 değerleri istatiksel olarak değerlendirildiğinde anlamlı olarak (p<0,05) saptandı. T1AG'de Sİ değeri geniş çapta ve odakta anlamlı (p>0,05) saptanmadı. Yalnızca T2 A yağ baskısız görüntülemede Sİ değeri geniş çapta anlamlı (p=0,03) çıkıp odakta (p=0,08) çıkmadı ve Woİ2 değeri odakta anlamlı (p=0,03) çıkıp geniş çapta (p=0,33) çıkmadı. DDBHRHK-YDBHRH'leri birbirinden ayırmada da engeniş çap ve odakta yapılan; T2 A yağ baskısız ve T1AG'deki Sİ özelliği, T1SİO, TDSİO, Wiİ1-2-3ve Woİ2 değerleri istatiksel olarak değerlendirildiğinde anlamlı olarak (p>,05) saptanmadı. Yalnızca T2SİO (p=0,03) ve Woİ1 değeri (p=0,04), geniş çapta anlamlı çıkıp odakta çıkmadı. Odakta yapılan ölçümlerin geniş çap ölçümlerine göre anlamlılık veren bir katkısı olmadı. Sonuç: Çalışmamızda kullandığımız T2 A yağ baskısız sekanslı görüntüler; T1 A serilerde in faz (IP)-opposed faz (OP), prekontrast ve kontrast madde verdikten sonra dinamik elde olunan görüntüleri göz önüne aldığımızda, bu görüntüler ile ele olunan T2 A yağ baskısız ve T1AH'deki Sİ özelliği, T1SİO, TDSİO, Wiİ1-2-3 ve Woİ1-2 değerleri ile, böbrek tümörlerinde histopatolojik tip belirleyebilmekteyiz. Kullandığımız tüm sekanslara ait görüntüler bize bu ayrımda fayda vermekte olup; yalnızca prekontrast elde olunan T1 A serilerdeki görüntülerde elde olunan ölçümler tek başına anlam vermemekte, lezyonun kontrast madde tutulumu ve kontrast maddeden yıkanması için prekontrast tetkikleri ile karşılaştırıldığından indirekt olarak tanı için yardımcı olmaktadır. BHRHK'lerde FDS'ye göre DDBHRHK- YDBHRHK'leri ayırabilmek için ise, sadece kitlelerin en uzun çap ortalamaları ile FDS'ye göre dereceleri arasında doğru orantı saptanmış olup kullanılan sekanslar ve bu sekanslarda elde olunan ölçümler arasında FDS'ye göre istatiksel olarak anlamlılık saptanmadı. Yalnızca tümörün en geniş çapında ölçülen T2SİO değeri (p=0,03) ve Woİ1 değeri (p=0,04) sınırda istatiksel olarak anlamlı bilgi vermektedir. Böbrek tümörlerinde, MRG' de kullandığımız tüm sekanslar ve bu sekanslardaki görüntülerle elde olunan ölçümler, RHK'lerde subtip ayrımı yapabilmek için ayrı katkı sağlamaktadır. Aim: In this study, we aimed to determine the histopathological type and Fuhrman grading of renal masses by using different sequences in Magnetic Resonance Imaging (MRI) before biopsy/operation. In the light of the information obtained, we aimed to determine the sequences which are important for mass characterization in order to shorten the duration of MRI acqusition, which is a radiation safe cross-sectional examination technique, and reduce the disturbance of the patient's comfort. And also we aimed to contribute to the management of the treatment plan associated with histopathological type before the biopsy/operation. Materials and Methods: This retrospective study includes evaluation of contrast enhanced dynamic upper abdomen MRI of 83 patients that were acquired between 2010-2019 with 1,5 Tesla MRI devices who underwent surgical treatment/biopsy for renal cancer. The parameters we used were; age and sex distribution of patients; size of the mass; T1-weighted imaging (T1WI) and T2-weighted imaging (T2WI) signal intensity (SI) characteristics; presence of intracellular fat; enhancement and washout patterns of areas within a region of interest (ROI) that is placed on contrast enhancing solid component lacking any necrosis/cyst/calcification and an axial plane where largest diameter of the mass is observed. Using this parameters, we aimed to differentiate Fuhrman - low and high grade histopathological subtypes of clear cell RCC (CCRCC) and in between renal cell carcinomas (RCC). Results: Of the 83 patients, 33 were females (39,8%) and 50 were males (60,2%). The mean age of the patients was 57,54 ± 12,63 years. There were 8 benign lesions including 4 angiomyolipomas (AML) and 4 oncocytomas; 75 malignant lesions including 42 CCRCC, 19 papillary RCC (PRCC) and 14 chromophobe RCC (CRCC) in our study. No significant association between tumours (benign vs. malignant lesions) and the mean age of patients (p = 0,14), the longest median diameter of the mass (p= 0,23), the mean endo-exophytic localization of the lesion (p = 1,00), the predominant component of the mass (p= 0,56) was found statistically. No significant association between tumours (CCRCC vs. nonclear cell RCC (NCCRCC)) and the mean age of patients (p= 0,57) and the longest median diameter of the mass (p = 0,77) and the mean endo-exophytic localization of the lesion (p = 0,62) and the predominant component of the mass (p = 0,84) was found statistically. No significant association between tumours (CCRCC, PRCC vs. CRCC) and the mean age of patients (p = 0,24) and the longest median diameter of the mass (p=0,86) and the predominant component of the mass (p= 0,19) was found statiscally. No significant association between tumour grade and the mean age of patients (p= 0,70) and the mean endo-exophytic localization of the lesion (p = 0,47) and the predominant component of the mass (p = 0,70) was observed statistically. Only one significant association between tumour grade and size (p = 0,01) found in multivariate analysis. A significant association between tumours (CCRCC vs. NCCRCC) in the largest component of the mass and ROI, SI characteristics of not-fat suppressed T2WI (T2SI) and T2 signal intensity ratio (T2SIR) and signal intensity index (SII), tumour-to-spleen ratio (TSR) and wash-in index1-2-3 (WiI-1: corticomedullary phase, 2: early nephrographic phase, 3: late nephrographic phase) and wash-out index1 (WoI- 1:early nephrographic phase, 2: late nephrographic phase) was found statistically (p<0,05). There was a significant association between SI characteristics of T1WI (T1SI) and tumours (CCRCC vs. NCCRCC) in the largest component of the mass, but not in the ROI (p=0,36). There was a significant association between WoI2 and tumours (CCRCC vs. NCCRCC) in the ROI but not in the the largest component of the mass (p=0,17) . A significant association between tumours (CCRCC, PRCC vs. CRCC) in the largest component of the mass and ROI, T2SIR and SII and TSR and WiI1-2-3 and WoI1 was found statistically (p<0,05). No association was found between T1SI anda RCCs in the largest component of the mass and ROI (p>0,05). A significant association in the largest component of the mass, T2SI was observed statistically (p=0,03) but not in the ROI (p=0,08). A significant association in the ROI, WoI2 was observed statistically (p=0,03) but not in the the largest component of the mass (p=0,33) . In the largest component of the mass and ROI, there was no significant association between tumour grade T2SI and SII and TSR and WiI1-2-3 and WoI2 was observed statistically (p>0,05). There were significant associations with T2SIR (p=0,03) and WoI1 (p=0,04) in the largest component of the mass but no significant association was found in the ROI. Conclusion: We used the sequences which are not-fat suppressed T2WI and T1WI with adding features in-phase, opposed-phase, before and after contrast agent and we calculated the values of T2SI, T1SI, SII, TSR, WiI1-2-3 and WoI1-2. All these findings provide us the distinction between histopathological type in renal tumours, except the precontrast images onT1WI alone. But this sequence is essential for evaluation of postcontrast images. There is only one significant association between tumour grade and diameter of the masses. The sequences used and the measurements obtained in these sequences were not statistically important. There was a borderline significant association between tumour grade and T2SIR (p=0,03) and WoI1 (p=0,04) statistically (p<0,05) in the largest component of the mass. In renal tumors, all the MRI sequences that we use and the measurements obtained with these images make a separate contribution to differentiation subtype in RCC.
