ÖZET Aksiller Yayılımı Olmayan Meme Kanserli Hastalarda HER2/Neu Gen Amplifîkasyonunun Floresan İn Situ Hibridizasyon (FISH) Yöntemi İle İncelenmesi HER2/neu (c-erbB2), son yıllarda meme kanserlerinin prognozunun belirlenmesi ve tedavi seçeneklerinin değerlendirilmesinde önemli bir parametre olarak ortaya çıkmıştır. HER2/neu değerlendirilmesi için çeşitli yöntemler mevcutsa da ilk basamak yöntem olarak immünhistokimya (IHK), altın standart olarak da floresan in-situ hibridizasyon (FISH) en çok üzerinde durulan yöntemlerdir. Bu çalışmada, doku mikroarray yöntemi ile hazırlanmış bloklar üzerinde 85 aksilla negatif meme kanseri olgusunda IHK ve FISH ile HER2/neu araştırılmıştır. Materyal Metod: 85 olguda parafin bloklardan elde edilen 4 mm. çaplı silindirler (2-5 silindir/olgu) toplam 25 parafin blokta toplanmıştır. Bunlardan hazırlanan kesitlerde streptavidin-biyotin yöntemiyle IHK' sal olarak (Dako) ve FISH ile (Q-Biogene) HER2/neu araştırılmıştır. Bulgular: Olguların sırasıyla % 35.2, % 25.9, % 20, %18.9'unda IHK 0, +1, +2, +3 olarak değerlendirilmiştir. FISH ile ise olguların % 15. 7' si amplifıkasyon açısından pozitif bulunmuştur. IHK ile 0, +1 olarak değerlendirilen olguların hiçbirinde amplifıkasyon saptanmamıştır. +2 olarak değerlendirilen 15 olgunun sadece birinde amplifıkasyon izlenmiştir. Buna karşılık +3 olarak değerlendirilen 12 olgunun % 83.3'ü amplifikasyonludur. +3 olarak değerlendirilen 2 olguda ise amplifıkasyon saptanmamıştır. En az 2 doku silindiri ile değerlendirilen 75 olgunun IHK' sal değerlendirmelerinin karşılaştırılmasında, sadece sırasıyla %52 ve %32 olguda tam ve orta derecede uyum saptanmıştır. Olgularımızın 4 tanesinde silindirler arasında hasta ile ilgili tedavi planını değiştirebilecek düzeyde önemli farklılık saptanmıştır. 55Tartışma ve Sonuç: 1. IHK ile değerlendirmede, +2 olguların büyük kısmı ve +3 olguların bir bölümü gerçekte amplifikasyon olmaksızın pozitif olarak değerlendirilmektedir. Bunların FISH ile teyidi gereklidir. 0 ve +1 olgularda negatifliğin FISH ile teyidi gerekli değildir. 2. Az miktarda tümör dokusu içeren küçük biyopsi materyallerinde immünhistokimyasal HER2/neu değerlendirilmesi tümör heterojenitesi nedeniyle risklidir. SUMMARY Evaluation of HER2/neu Gene Amplification by Fluorescence in Situ Hybridization (FISH) in Patients with Axillary Lymph Node Negative Breast Carcinoma HER2/neu (c-erbB2) is an important prognostic parameter in node positive breast carcinoma patients and it is also helpful in the selection of therapeutic regimen. Although there are various methods for evolution of HER2/neu, screening method is immunohistochemistry (IHC) and the gold standard is fluorescence in situ hybridization (FISH). The aim of this study is to determine HER2/neu status by IHC and FISH at 85 tumors from axillary lymph node negative breast carcinoma patients, using low density manual tissue microarray and to correlate the results with clinicopathological parameters. Materials and Methods: For immunohistochemical study and FISH analysis, tissue cylinders of 4 mm diameter (2-5 cylinders/case) from paraffin blocks were arrayed in 25 recipient paraffin blocks. We analyzed HER2/neu status of 85 invasive breast carcinomas by IHC (DAKO), using streptavidin biotin immunodetection system and by FISH (Q- biogene). Results: Immunohistochemically, 35.2%, 25.2%, 20% and 18.9% of 85 cases were scored as 0, +1, +2 and +3 respectively. HER2/neu was investigated by FISH at 70 of 85 cases. Amplification was obtained at 15.7% of these cases. There is no evidence of HER2/neu gene amplification by FISH at cases scored 0, +1 by IHC. Only 1/15 IHC +2 tumors was FISH positive. 83.3% of 12 IHC +3 patients was positive by FISH. However, 2 cases that were scored as +3 by IHC, were found to be negative by FISH. 75 cases were evaluated by at least two tissue cylinders (2-5 cylinders/case) and immunohistochemical scores of different cylinders of the same tumor were compared. In only 52% of the cases same immunohistochemical score was observed 57in all tissue cylinders from the same case. In four cases, there was important discordance between the scores of cylinders that could affect the treatment regimen of the patients. Discussion and Conclusion: 1- Our study showed that most of +2 cases and a small percentage of +3 cases are evaluated immunohistochemically positive without amplification. These cases must be confirmed by FISH. FISH confirmation is not necessary in score 0 and +1 cases. 2- Evaluation of HER2/neu by IHC may be risky at small biopsy specimens because of tumor heterogeneity.
