Giriş: Çalışmamızda, acil servise 24 saat içinde tekrar başvuran hastaların ilk başvuru zamanının, geri dönüşler üzerine olan etkisinin tespiti birincil amaç olarak belirlenmiştir. Eğer var ise, bu etkinin ne ölçüde malpraktise yol açtığının araştırılması ikincil amaçtır. Materyel ve Metod: Çalışmamız Ümraniye Eğitim ve Araştırma Hastanesi acil servisine 30.09.2015-30.09.2016 tarihleri arasında başvuran hastalar ile kesitsel ve retrospektif olarak gerçekleştirildi. Belirlenen tarih aralığında 24 saat içerisinde hastanemiz acil servisine tekrar başvuru yapmış hastalar; yaş, cinsiyet, ilk başvuru tanısı, ikinci başvuru tanıları, tanıların ilgili oldukları bölümler, iki başvuru arasındaki süre, hastaların başvuru zamanları ve sonlanımları incelendi. Bulgular: Mükerrer başvuruların yaş ortalaması 27,2±20,8 (%95 GA:26,3-28,1) yıl olup, %51'i kadındı. Mükerrer başvuru sıklığı %0,35 olarak saptandı.. Ortalama mükerrer başvuru süresi 16:17±6:15 (%95 GA:16:01-16:33) saatti. İlk başvurunun %46,4'ü, mükerrer başvuruların %53,6'sı mesai içindeydi. İlk ve mükerrer başvuru sıklığı mesai dışında olan hastaların sayısı en fazlaydı (p<0,05). Hastaların ilk ve mükerrer başvuru sırasında doğum şüphesi (%16,2) ve renal kolik/üriner sistem enfeksiyonu (%15,5) en sık görülen iki tanıydı. İkinci başvuruda hastaların %10'u yatırılırken, %0,2 başka merkeze sevk edildi. Kadın doğum ve genel cerrahi ile ilgili patolojileri olan hastaların yatış gereksinimleri anlamlı olarak yüksekti (p<0,05). Çalışmamızda ilk başvuru zamanının, yatış gereksinimi arasında ilişki saptanmazken (p>0,05), ikinci başvurda mesai saatleri dışında yatış oranları anlamlı olarak yüksekti (p<0,05). İkinci başvurusu mesai dışında olan kadın doğum hastalarının yatış sıklıkları yüksekti (p<0,05). Diğer bölümleri ilgilendiren patolojilerin başvuru süresi ile ilişkisi saptanmadı (p>0,05). Sonuç : Acil servise mükerrer olarak getirilen hastaların muayenelerinin daha dikkatli, tetkik seçimin daha itinalı ve takiplerinin daha düzenli yapılması gerektiği kanısındayız. Gereksinim halinde tanıları netleşmeyen hastaların takip süreleri uzatılarak, mesai saatleri içerisinde bu hastaların tekrar değerlendirilmesi önerilebilir.
Aim: In our study primarily, we aimed to measure the effect of first attendance time of the patients to estimate the revisit of emergency clinic in 24 hours. Secondarily, how 24 hour revisit leads to malpractice is evaluated if so. Materials and Methods: Our study was performed cross-sectional and retrospectively between the dates of 30.09.2015-30.09.2016 in Ümraniye Training and Research Hospital Emergency Department. Patients who had revisited, emergency room within 24 hours of the specified date range, were examined in terms of age, gender, first referral diagnosis, second referral diagnosis, Medical specialities in which the diagnoses were related, duration between two referrals, patient referral times and outcomes. Results: The mean age of the revisits was 27.2 ± 20.8 (95% CI: 26.3 to 28.1) years, 51% patients were female. The frequency of 24 hour revisits was found to be 0.35%. The average duration between two visits by the same patients in 24 hours was 16: 17 ± 6: 15 (95% GA: 16: 01-16:33). 46.4% of the first visit and 53.6% of the 24 hour revisits were in working hours. Patients were mostly applied to emergency clinic out of working hours both first and second(p<0,05). The most common two diagnoses were birth evaluation(16,2%) and renal colic / urinary system infection (15,5%) during the first and 24hour revisits. On the second visit, 10% of the patients were hospitalized and 0.2% were referred to another center. Patients with clinical conditions related to gynecology and general surgery had significantly higher hospitalization rate (p<0,05). In our study, while there was no relationship between the first visit time and the need for hospitalization (p> 0.05), the second visits in 24 hours significantly resulted with hospitalization p> 0.05).The rates of hospitalization outside working hours were significantly higher in the second visit (p<0,05). Gynecologic patients whose second visit was out of working hours had higher incidence of hospitalizastion (p<0,05). The clinical diagnosis related to other clinical subbranches were not related to the visit time (p> 0.05). Conclusion: we believe that patients who have revisited emergency room should be examined more carefully, the choice of diagnostic work-up should be more detailed, and follow-up should be more regular. If necessary, patients whose diagnosis is unclear can be put on prolonged follow-up period and reevaluation of these patients during working hourscan be required.
