Ameliyat sonrası ileus, kolorektal cerrahi girişimlerden sonra bağırsak motilitesinin geçici şekilde kesintiye uğraması sonucu ortaya çıkmaktadır. Ameliyat sonrası ileus, ağrı, bulantı-kusma ve komplikasyon sıklığında artış benzeri olumsuz hasta sonuçlarına yol açması nedeniyle önlenmelidir. Sakız çiğneme, ameliyat sonrası ileusun önlenmesi amacıyla kullanılan yöntemler arasındadır. Bu araştırma, kolorektal cerrahi girişim sonrası ortaya çıkan ileusun önlenmesinde sakız çiğnemenin etkisini belirlemek üzere deneysel olarak planlandı ve uygulandı. Trakya Üniversitesi, Sağlık Uygulama ve Araştırma Merkezi, Genel Cerrahi Anabilim Dalı'nda Kasım 2011-Aralık 2012 tarihleri arasında cerrahi girişim gerçekleştirilen hastalar arasından örneklem seçim kriterlerini karşılayan ve araştırmaya katılmayı kabul eden hastalar araştırmanın örneklemini (n=60) oluşturdu. Literatür doğrultusunda oluşturulan "Veri Toplama Formu" ile elde edilen veriler ortalama, ortanca, sayı, yüzdelik, korelasyon analizi, Mann-Whitney U, ki-kare ve t-testi ile değerlendirildi. Araştırmada, ameliyat sonrası gaz ve gaita çıkarma zamanının deney grubunda daha kısa olduğu (p<0,001), deney grubundaki hastaların ameliyat sonrası daha kısa süre içerisinde taburcu oldukları (p<0,05), hastaların ameliyat sonrası beslenmeye başlama zamanı kısaldıkça gaz çıkarma zamanının da kısaldığı (p<0.05), sakız çiğnemenin ilk gaz ve gaita çıkarma zamanını etkileyen bağımsız değişken olduğu (p<0,001) bulundu. Sonuç olarak, sakız çiğneme kolorektal cerrahi girişim sonrası ilk gaz ve gaita çıkarma zamanını kısaltarak ameliyat sonrası ileusun önlenmesine yardımcı olmaktadır. Kolorektal cerrahi girişim geçiren hastaların boşaltım aktivitesine yönelik hemşirelik bakım uygulamaları kapsamında, sakız çiğnemeye yer verilerek ameliyat sonrası ileus gelişme riski önlenebilir. Bu riskin fazla olduğu diğer cerrahi girişimlerden sonra sakız çiğnemenin etkisini inceleyen araştırmalar yapılması önerilebilir.
Postoperative ileus occurs after colorectal surgery as a result of temporary interruption of bowel motility. Prevention of postoperative ileus is important because of it caused negative patient outcomes such as pain, nausea, vomiting and increasing of complications. Gum chewing is among the methods used to preventing postoperative ileus. This research was planned and applied in order to determine the effect of gum chewing on prevention of postoperative ileus. The patients who underwent colorectal surgery at Trakya University, Health Research and Practice Center, General Surgery Department between the dates of November 2011 and December 2012, volunteers the research and meets the sample criteria (n=60) were included in the research. Data obtained by "Data Collection Form" created in accordance with the literature was evaluated by mean, median, frequency, percentage, correlation analysis, Mann-Whitney U test, chi-square test and t-test. In the research, it was found that first flatus and defecation time after the surgery were shorter (p<0,001) and patients were discharged in a shorter time (p<0,05) in the experimental group; when patients' feeding time got shorter, their first flatus time also got shorter (p<0,05); gum chewing was the independent factor that effect the first flatus and defecation time (p<0,001). As a result, gum chewing helps preventing postoperative ileus after colorectal surgery by shortening the first flatus and defecation time. Within the scope nursing practices for elimination activity of the patients undergone colorectal surgery, postoperative ileus development risk can be prevented by giving place to the chewing gum. The studies that examine the effectiveness of gum chewing on the other surgical interventions in which the development risk of postoperative ileus should be performed.
