Mantar enfeksiyonlarında en çok izole edilen türün Candida albicans olduğu literatürce tespit edilmiştir. Bizim çalışmamızda en çok izole edilen tür Cryptococcus laurentii olmuştur. Son yıllarda tüm dünyada sentetik ilaçların gelişigüzel kullanılması, immün sistemi zayıflatan hastalıkların tedavisinin zorlaşması, sentetik ilaçların yan etkilerinin yeni hastalıklara öncülük etmesi, maya enfeksiyonlarına neden olan etkenlerin insidansının artması ve mevcut antifungal ilaçlara direnç gelişmesi bitkisel kaynaklardan antifungal bileşiklerin araştırılması için bilim adamlarını teşvik etmiştir. Bu nedenle bu çalışmada bitki ekstraktlarının antifungal etkileri araştırılarak mevcut antifungallerden daha etkili yeni antifungallerin bulunması amaçlanmıştır. Mantar enfeksiyonu olan hastalardan alınan kan, idrar, gaita, vajen, serviks, balgam, tırnak ve deri döküntüsü örneklerinden 40 adet izolat elde edilerek API 20C AUX (biomerieux) sistemi ile tanımlanmıştır. Tanımlanan 40 adet maya izolatının %42,5'i Cryptococcus laurentii, %32,5'i Candida albicans, %15'i Candida kefyr, %7,5'u Candida tropicalis, %2,5'u Candida lusitaniae olduğu saptanmıştır. Çalışmamız Türkiye'de yapılan diğer çalışmalarla karşılaştırıldığında farklılık göstermektedir. Bu çalışmada %42,5 oranıyla en çok izole edilen tür Cryptococcus laurentii olmuştur. Cryptococcus laurentii, Candida albicans, Candida kefyr, Candida tropicalis, Candida lusitaniae izolatlarının oceral ve terbisil antifungal ilaçlarına dirençlilik durumları agar kuyucuk difüzyon metodu ile belirlenmiştir. Antifungallere en çok direnç gösteren izolatlar Candida albicans M7, Candida albicans M10, Candida albicans M13, Candida tropicalis M15, Cryptococcus laurentii M27 ve Cryptococcus laurentii M39 olduğu saptanmıştır. Ankara Düzen Laboratuvarında MALDI-TOF MS yöntemiyle dirençli izolatların identifikasyonları doğrulanmıştır. Bu dirençli izolatlar üzerinde ülkemizde doğal olarak yetişen Hibiscus sabdariffa, Ceratonia siliqua, Origanum minutiflorum, Cotinus coggygria ve Lavandula stoechas bitki ekstraktlarının antifungal aktiviteleri incelenmiştir. Yapılan antifungal denemeler sonunda dirençli izolatlara karşı yüksek antifungal aktivite gösteren ekstraktın Hibiscus sabdariffa ve Ceratonia siliqua olduğu tespit edilmiştir. Cryptococcus laurentii M39 ve Cryptococcus laurentii M27 izolatlarının tamamını öldüren Hibiscus sabdariffa konsantrasyon miktarları 5 mg/ml'dir. Hibiscus sabdariffa ekstraktının Cryptococcus laurentii M39 ve Cryptococcus laurentii M27 izolatları için LC50 değerleri sırasıyla 0,6 ve 1,0 mg/ml olarak bulunmuştur. Candida albicans M7, Candida albicans M10, Candida albicans M13 ve Candida tropicalis M15 izolatlarının tamamını öldüren Ceratonia siliqua konsantrasyon miktarları sırasıyla 10, 7,5, 5 ve 10 mg/ml'dir. Ceratonia siliqua ekstraktının Candida albicans M7, Candida albicans M10, Candida albicans M13 ve Candida tropicalis M15 izolatları için LC50 değerleri sırasıyla 2,2, 1,8, 2,0 ve 2,0 mg/ml olarak bulunmuştur. Bu çalışmada Hibiscus sabdariffa ekstraktının ve Ceratonia siliqua ekstraktının dirençli izolatlara karşı antifungal deneylerde kullanılan ilaçlardan daha yüksek etki gösterdiği saptanmıştır. Özellikle Cryptococcus laurentii kaynaklı fungal hastalıkların tedavisi için spesifik inhibitörlerin olmadığından bu çalışmanın yeni antifungallerin üretimine öncülük edeceği düşünülmektedir.
It has been determined by the mostly isolated species in fungal infections is the Candida albicans. In our study the mostly isolated species was Cryptococcus laurentii. Recently all around the world indiscriminate usage of synthetic drugs, difficulty in treatment of diseases that weaken the immun system, side effects of synthetics drugs leading to new diseases, the increased incidence of factors that cause yeast infections, development of resistance to current antifungal drugs encourage the scientist to search antifungal compounds from herbal sources. Therefore, in that study by searching antifungal effects of plant extracts it is aimed to find new antifungal drugs that is more effective than current antifungal medicine. 40 new isolates are obtained from blood, urine, faeces, vagina, cervix, sputum, nail and skin rashes of patients with fungal infections and identified with API 20C AUX system. It was determined that 40 yeast isolates are composed of %42,5 Cryptococcus laurentii, %32,5 Candida albicans, %15 Candida kefyr, %7,5 Candida tropicalis and %2,5 Candida lusitaniae. Compared with other studies in Turkey, our study has differences. In our study most isolated species is Cryptococcus laurentii with 42,5 percentage. Resistance of Cryptococcus laurentii, Candida albicans, Candida kefyr, Candida tropicalis, Candida lusitaniae isolates to antifungal drugs as oceral and terbisil was determined by agar well diffusion method. It was determined that isolates having more resistance to antifungal drugs are Candida albicans M7, Candida albicans M10, Candida albicans M13, Candida tropicalis M15, Cryptococcus laurentii M27 and Cryptococcus laurentii M39. The resistant isolates was confirmed with MALDI-TOF MS method in Ankara Düzen Laboratories. The antifungal activity of herbal extracts that grown naturally in our country as Hibiscus sabdariffa, Ceratonia siliqua, Origanum minutiflorum, Cotinus coggygria and Lavandula stoechas were examined on the resistant isolates. At the result of antifungal assays, the extract which has higher antifungal activity upon resistant isolates are determined as Hibiscus sabdariffa and Ceratonia siliqua. The minimum concentration of Hibiscus sabdariffa that kills Cryptococcus laurentii M39 and Cryptococcus laurentii M27 isolates is 5 mg/ml. LC50 values of Hibiscus sabdariffa extracts for the Cryptococcus laurentii M39 and Cryptococcus laurentii M27 isolates are 0,6 and 1,0 mg/ml respectively. The minimum concentration of Ceratonia siliqua that kills all of the Candida albicans M7, Candida albicans M10, Candida albicans M13 and Candida tropicalis M15 isolates are 10, 7,5, 5 ve 10 mg/ml respectively. LC50 values of Ceratonia siliqua extracts for the Candida albicans M7, Candida albicans M10, Candida albicans M13 and Candida tropicalis M15 isolates are 2,2, 1,8, 2,0 and 2,0 mg/ml. In this study it was determined that Hibiscus sabdariffa and Ceratonia siliqua extracts have greater influence on resistant isolates than drugs used in antifungal experiments, and because there is no specific inhibitors for the treatment of fungal diseases caused by Cryptococcus laurentii, that study could be lead to production of new antifungal drugs.
