Tüm anestezi uygulamalarının %10-20'si ürolojik girişimler için yapılır. Ürolojik girişimlerde topikal, rejyonel ve genel anestezi teknikleri tercih edilebilir. TUR-P yapılan hastalarda TUR-P sendromu geliştiği takdirde hastada oluşabilecek mental durum değişikliklerini gözlemlemeye de imkan tanıdığı için çalışmamızdaki tüm hastalarda anestezi yöntemi olarak rejyonel anesteziyi, rejyonel anestezi olarak da spinal anesteziyi tercih ettik. İki gruba ayrılan hastalardan bir gruba perioperatif sıvı idamesi için %0,9 NaCl, diğer gruba ringer laktat solüsyonu verilerek metabolik etkilerinin karşılaştırılmasını amaçladık.Çalışmamızı Ankara Üniversitesi Tıp Fakültesi İbni Sina Hastanesi üroloji ameliyathanesinde TUR-P yapılan 54-80 yaşları arası, ASA I-III grubunda yer alan 26'şar kişilik iki grupta toplam 52 erkek hasta ile gerçekleştirdik. KOAH veya bilinen kalp yetmezliği olan hastalar, böbrek yetmezliği olan hastalar ve metabolik asidozu olan (BE>-4) hastalar çalışma dışı bırakıldı. Standart D II derivasyonunda EKG, puls oksimetre ile periferik oksijen saturasyonu (SpO2) ve noninvazif kan basıncı monitörizasyonu yapılan hastalara belirlenen sıvılardan biri damar yolundan 5-10 ml/kg/saat hızıyla verildi. Lokal anestezi ile radiyal arter kanülasyonu yapılarak arteriel kan gazı (AKG) örneği alındı. Bazal ve her 15 dakikada bir sistolik, diastolik ve ortalama arteriel kan basınçları kaydedildi. İşlem boyunca her 30 dakikada bir, cerrahi sonunda ve postoperatif 6. saatte AKG alındı. Bazal ve postoperatif 6. saatte alınan biyokimya değerleri kaydedildi. İşlem süresince 15 dakikada bir kan basınçları ve kalp hızı kaydedildi.Bu çalışmada iki grup arasında farklı değişkenlerden yaş, cerrahi süre, prostat büyüklüğü, operasyon boyunca verilen toplam sıvı ve operasyon boyunca kullanılan toplam irrigasyon sıvısı yönünden istatistiksel olarak anlamlı fark bulunmamıştır. Hemodinami açısından her iki grup da genel olarak birbirine benzerdi. Kan üre azotu (BUN), kreatinin, protein, albumin, hemoglobin, pH, BE'nin bazal ve belirlenen zamanlardaki değerleri arasında istatistiksel olarak anlamlı fark bulunmamıştır. Na+ değeri 30. dk'da NaCl grubunda, K+ değeri postoperatif 6. saatte RL grubunda, Cl¯ değeri cerrahi sonu ve postoperatif 6. saatte NaCl grubunda, laktat 30. dk'da RL grubunda, PaCO2 değeri 30. dakika NaCl grubunda, HCO3¯ değerinde postoperatif 6. saatte RL grubunda istatisriksel olarak daha yüksekti.Sonuç olarak TUR-P yapılacak hastalarda hem %0,9 NaCl hem de Ringer Laktat'ın güvenle kullanılabileceği, buna rağmen her hastanın ayrı değerlendirilerek preoperatif, intraoperatif ve postoperatif klinik durumu ve laboratuar verilerine göre karar verilmesinin uygun olacağı kanaatindeyiz.
10-20% of all anesthesia uses are performed for urological operations. In urological operations, topical, regional and general anesthesia techniques may be preferred. In patients operated with TUR-P, we preferred regional anesthesia for all patients in our study as anesthesia method and we preferred spinal anesthesia as regional anesthesia. If TUR-P syndrome develops, this method allows us to observe the mental status alterations which may occur in the patient. Of the patients separated into two groups, we intended to compare the metabolic effects by giving NaCI 0, 9% for preoperative fluid maintenance in one group and ringer lactate solution in other group.We carried out our study with total 52 male patients from two groups in which each contains 26 people who were between the ages of 54-80, in ASA I-III group and operated with TUR-P in urology operating room of Ankara University Medical Faculty Ibni Sina Hospital. We exempted the patients having Chronic Obstructive Lung Disease or common coronary failure, kidney failure and metabolic acidosis (BE>-4) from the study. One of the specified fluids was transfused to the patients applied EKG in standard D II derivation, peripheral oxygen saturation (SpO2) with pulse oximeter and noninvasive blood pressure monitorization at the rate of 5-10 ml/kg/hour. Arterial blood gas (ABG) sample was taken by radial artery cannulation with local anesthesia. Basal and in every 15 minutes systolic, diastolic and average arterial blood pressures were recorded. Arterial blood gas (ABG) was taken in every 30 minutes during the operation, at the end of the operation and in the 6th hour postoperatively. Basal and biochemistry values obtained in the 6th hour postoperatively were recorded. During the operation, blood pressures and heart rate were recorded in every 15 minutes.In this study, it is found that there was no statistically significant difference between two groups in terms of age, surgery period, prostate size, total fluid given through the operation and total irrigation fluid used thorough the operation. Both groups were generally similar in terms of hemodynamic. It is found that there was no statistically difference between the basal and certain times values of blood urea nitrogen (BUN), creatinine, protein, albumin, hemoglobin, pH and BE. Na+ value in 30th minute in NaCI group, K+ value in the postoperative 6th hour in RL group, CI- value at the end of the operation and postoperative 6th hour in NaCI group, lactat in 30th minute in RL group, PaCO2 value in 30th minute in NaCI group, HCO3 value in postoperative 6th hour in RL group was statistically higher.Consequently, both 0,9% NaCI and Ringer Lactate can be used safely in patients to be operated with TUR-P, nevertheless we consider that decision making according to the preoperative, intraoperative and postoperative clinical condition and laboratory data by evaluating the every patient separately will be suitable.
