İnme çocuklarda giderek artan sıklıkta tanı almakta olan, morbidite ve mortalitesi yüksek bir hastalıktır. Ocak 2000-Aralık 2011 tarihleri arasında pediatrik hematoloji ve nöroloji polikliniklerinde tetkik ve tedavi edilmiş ve servislerde yatarak izlenmiş ilk atak inme olgularını retrospektif olarak değerlendirdik. Çalışmaya arteriyel iskemik inme (Aİİ), hemorajik inme (Hİ) ve serebral sinovenöz tromboz (SVT) tanılı olgular dahil edildi. İstatistiksel analizler için NCSS (Number Cruncher Statistical System) 2007&PASS (Power Analysis and Sample Size) 2008 Statistical Software (Utah, USA) programı kullanıldı. Niteliksel verilerin karşılaştırılmasında ise Pearson Ki-Kare testi ve farklılığa neden çıkan grubun tespitinde Fisher?s Exact test ve Yates Continuity Correction test kullanıldı. Aİİ tanılı olgular çalışma grubumuzun çoğunluğunu oluşturmakta (n=69, %58,5) etiyolojide en sık kardiyak nedenler yer almaktadır (n=12, %17). Hİ olguların %19,5?inde (n=23), etiyolojide en sık yenidoğanın geç hemorajik hastalığı (n=10, %43), SVT olguların %22?sinde (n=26) ve etiyolojide en sık akut otitis media-mastoidit (n=7, %27) vardı. Kardiyak hastalığı olan Aİİ tanılı olguların yarısında kompleks konjenital kalp hastalığı, diğer olgularda Fallot tetralojisi (n=2), romatizmal mitral kapak hastalığı (n=1), dilate kardiyomiyopati (n=1), triküspit atrezisi (n=1), aort koarktasyonu (n=1) mevcuttu. Travma hikayesi Aİİ olgularının %13?ünde (n=9), romatizmal hastalıklar (Sneddon sendromu, sarkoidoz, fibromuskuler displazi, Takayasu arteriti, primer serebral vaskülit) %7,5?inde (n=5) vardı. Behçet hastalığı SVT olgularının %14?ünde (n=4) altta yatan nedendi. Behçet hastalığı birden fazla venöz sinüs tutulumu ile seyredebilen, Türkiye?de SVT etiyolojisinde önemli bir hastalıktır. Başvuruda en sık saptanan bulgular `hemipleji? (n=57, %48,3) ve `konvülziyon? (n=43, %36,8) idi. Diğer bulgular; fasyal sinir felci (n=14, %11,8), afazi (n=12, %10,1), baş ağrısı (n=24, %20,3), kusma (n=9, %7,6), bilinç değişikliğiydi (n=8, %6,7). Hemipleji sıklıkla Aİİ grubunda görülmekteyken, SVT olguları çoğunlukla başağrısı ve konvülziyon ile başvurmuştu. İnme ile ilişkilendirilmiş pek çok protrombotik durum tariflenmiştir. Çalışma grubumuzda olguların %15?inde (n=9) faktör V Leiden mutasyonu, %3,3?ünde (n=2) protrombin G20210A gen mutasyonu, %43,1?inde (n=25) MTHFR C677T mutasyonu ve %12,3?ünde (n=16) MTHFR A1298C mutasyonu vardı. İnme alt tipleri ile genetik mutasyonlar arasında istatistiksel olarak anlamlı fark saptanmadı. Olguların %47,4?ünde (n=56) nörolojik defisit vardı. Nörolojik defisit ve epilepsi genç hastalarda daha sıktı. Toplam 95 Aİİ ve SVT tanılı olgudan 52?si (%53,6) antitrombotik tedavi (düşük molekül ağırlıklı heparin, anfraksiyone heparin, varfarin) almış olup, tedaviye bağlı ciddi komplikasyon saptanmadı. Konjenital kalp hastalığı olan Aİİ tanılı bir hasta ile Makrosefali-Telanjiektazi sendromu tanılı bir SVT olgusu tromboembolik olay sonrası erken dönemde kaybedildi. Kliniğimizde antikoagülan tedavide sıklıkla DMAH kullanmaktayız ve çalışma grubumuzda hiçbir hastada tedaviye bağlı ciddi kanama saptanmadı. Mortalite oranımızın düşük olması yaşayan olguların tarafımıza sevk edlmesine bağlı olabilir.
Stroke has been recognized more frequently in children and represents a major cause of morbidity and mortality. We reviewed the medical records of cases with first-ever pediatric stroke in both pediatric neurology and pediatric hematology department in our center between January 2000-December 2011. Patients were eligible for analysis if they had been diagnosed with arterial ischemic stroke (AIS), hemorrhagic stroke (HS) or cerebral sinovenous thrombosis (CSVT). NCSS (Number Cruncher Statistical System) 2007&PASS (Power Analysis and Sample Size), 2008 Statistical Software (Utah, USA) were used for statistical analysis. Pearson ?² test, Fisher?s Exact test and Yates Continuity Correction were used to exam relationships between stroke types, gender, age, neuroradiological images, treatment and prothrombotic disorders. AIS accounted for the majority of cases (n=69, 58.5%) and the major etiology was cardiac diseases (17%), HS accounted for 19.5% (n=23) and late hemorrhagic disease of the newborn was the major etiology (43%, n=10), CSVT accounted for 22% (n=26) of the cases and the major etiology was otitis media-mastoiditis (27%, n=7). In AIS group about half of the patients with cardiac diseases had complex congenital heart lesions and the other cardiac diseases in this group were Fallot tetralogy (n=2), rheumatic mitral valve disease (n=1), dilated cardiomyopathy (n=1), tricuspid atresia (n=1), aortic coarctation (n=1). Trauma history was found 13% (n=9) in patients with AIS and rheumatic diseases (Sneddon syndrome, sarcoidosis, fibromuscular dysplasia, Takayasu arteritis, primary cerebral vasculitis) were found in 7.5% of AIS group. Behcet?s disease was the etiology in 14% (n=4) patients with CSVT. Behcet?s disease is an important clinical entity for CSVT in Turkey and commonly effects multiple venous sinuses. The most frequent finding at presentation was `hemiplegia? (n=57, 48.3%) and `convulsion? (n=43, 36.8%). The other symptoms were; facial nerve palsy (n=14 , 11.8%), aphasia (n=12, 10.1%), headache (n=24, 20.3%), vomiting (n=9, 7.6%) and unconciousness (n=8, 6.7%). Hemiplegia was more common in the AIS group (n=48, 69.6%). Childen with CSVT commonly presented with headache or seizures. Various hypercoagulable states have been documented in childen with ischemic stroke. We identified 15% (n=9) of patients factor V Leiden mutation, 3.3% (n=2) prothrombin gene mutation (G20210A), 43.1% (n=25) MTHFR C677T mutation and 12.3% (n=16) MTHFR A1298C mutation. Any statistical relationship between stroke subtypes and gene mutations were not found. Neurological sequel was found 47.4% (n=56) of the cases. Neurological deficits and epilepsy were more common in younger patients. In total, 52 (53.6%) of the patients with CSVT and/or AIS received antithrombotic treatment (low molecular weight heparin, unfractionated heparin, warfarin); no major complications were observed. Two patients expired due to thrombosis; 1 had AIS and congenital heart disease and the other had CSVT and macrocephaly telangiectasia syndrome. We reviewed our experience of 12 years in diagnosis and treatment of pediatric stroke. We mainly use LMWH for anticoagulation and in our experience none of our patients had hemorrhage due to the treatment. We had a low mortality rate probably because only the survivors were referred to our center.
