ÖZET
Amaç: Akdeniz Üniversitesi Tıp Fakültesi Çocuk Metabolizma polikliniğinde
takipli olan FKÜ'lü hastaların uzun dönem diyet tedavisine bağlı olarak kemik
dansitometrilerini ve bu duruma yönelik risk faktörlerini belirlemektir.
Yöntem: Araştırmada Akdeniz Üniversitesi Tıp Fakültesi Çocuk Metabolizma
Bölümünde FKÜ tanısı ile takipli hastaların kemik dansitometrileri ile kalsiyum,
fosfor, magnezyum, ALP, D-vitamini düzeyi, PTH, anne sütü alım süresi, kan FA
düzeyinin ilişkisinin değerlendirilmesi amaçlandı ve araştırma retrospektif olarak
yapıldı.
Bulgular: Çalışmaya 32 FKÜ hastası dahil edilmiştir. Bunların 14'ü erkek
(%43,7) 18'i kızdı (%56,3). Bu 32 hastanın 18(%56,3)'inin kemik mineral
yoğunluğu normal bulunurken 14(%43,7)'ünün kemik mineral yoğunluğu düşük
bulundu. 14 hastanın ise 8'i osteopenik 6'si osteoporotik olarak saptandı. Bu
bulgular Akdeniz Üniversitesi Tıp Fakültesi Çocuk Metabolizma polikliniğinde
takipli FKÜ hastalarının %43,8'ünde kemik mineral yoğunluğu düşüklüğü
olduğunu gösterdi. Çalışmaya katılan hastaların anne sütü alım sürelerine
bakıldığında 20 hasta 4 ayın altında, 12 hasta ise 4 ayın üstünde anne sütü alım
süresi saptandı. Anne sütü alım süresi ile kemik mineral yoğunluğu arasında ilişki
saptanmadı. Hastaların geriye dönük olarak kalsiyum, fosfor, ALP, D-vitamini,
PTH ve kan FA düzeyleri tarandı. Kalsiyum, fosfor, ALP, D-vitamini, PTH ve
kan FA düzeyleri ile kemik dansitometri yoğunluğu arasında ilişki saptanamadı.
Sonuç: Birçok çalışma FKÜ'lü çocukların kemik yapısının etkilendiğini kabul
etse de henüz bu konuda ciddi bir bilgi açığı ve görüş ayrılığı vardır. Her ne kadar
uzun süreli diyet tedavisi almış FKÜ'lü hastalarda kemik mineral düşüklüğü
gelecek adına endişe verici gibi görünse de FKÜ diyetinin kemik gelişimi üzerine
etkileri, kemik durumunu gösteren biyokimyasal belirteçlerin ölçümlerinin
güvenilirliği ve kırık riskinin somut tahminleri ile ilişkili bir biyokimyasal belirteç
veya demografik veri saptayamadık. Bu konuda daha geniş hasta serilerinde
yapılacak çalışmalara ihtiyaç vardır.

ABSTRACT
Aim: The aim of the study is to evaluate the bone densitometry test outcomes
after long-term diet treatment and to identify the risk factors associated with this
disorder in patients with phenylketonuria (PKU) followed at the Pediatric
Metabolism Clinic of Akdeniz University Medical Faculty.
Method: The objective of the study was to evaluate the association of the bone
densitometry test outcomes retrospectively with several parameters including the
duration of breastfeeding and the levels of calcium, phosphorus, magnesium,
alkaline phosphatase (ALP), vitamin D, parathyroid hormone (PTH), and
phenylalanine (PA) in the patients followed-up at the Pediatric Metabolism Clinic
of Akdeniz University Medical Faculty.
Results: 32 patients with PKU were included in the study. Of these patients, 14
(43.7%) were males and 18 (56.3%) were females. The bone mineral densities of
these 32 patients were normal in 18 (56.3%) patients, whereas, they were lower
than normal limits in 14 (43.7%) patients. Of the latter 14 patients, osteopenia was
identified in 8 patients and osteoporosis was identified in 6 patients. These
findings demonstrated that the bone mineral density levels were low in 43.8% of
the PKU patients who were admitted for follow-up at the Pediatric Metabolism
Clinic of Akdeniz University Medical Faculty. The duration of breastfeeding
periods was found to be less than 4 months in 20 patients, however, 12 patients
had received breastfeeding for more than 4 months. There were no associations
between the duration of breastfeeding and the bone mineral density findings. The
levels of calcium, phosphorus, ALP, vitamin-D, PTH, and blood PA were
screened retrospectively. However, there were no associations between the bone
densitometry test findings and the levels of calcium, phosphorus, ALP, vitamin-D,
PTH, and the blood levels of PA.
Conclusion: Although it has been confirmed by studies that the bone structure is
affected in children with PKU, there is still a considerable lack of information and
debate on this issue. Despite our attempts to provide data to correspond to the
concerns on the potential further consequences of the low bone mineral levels in
the PKU patients treated with long-term PKU diet, we could not identify any
biochemical markers or demographic parameters associated with the effects of the
PKU diet on the bone development, the reliability of the biochemical markers, and
the objective predictions to determine the risks for bone fractures. Our findings
indicate that further studies are required to evaluate this issue including larger
patient series.
