Büllöz pemfigoid 60 yaşın üstünde, büllerle seyreden, otoimmun bir hastalıktır. Avrupa'da otoimmün büllü hastalıklar içinde en sık görülen hastalıktır. Patogenezde 180kDa ağırlığında BP2 ve 230kDa ağırlığında BP1 antijenlerine karşı gelişen otoantikorlar sorumlu tutulmuşturÇalışmamızda 1987-2007 yılları arasında İstanbul Üniversitesi İstanbul Tıp Fakültesi Dermatoloji Anabilim Dalı Büllü Hastalıklar Polikliniği'nde Büllöz pemfigoid tanısıyla izlenen 82 hasta retrospektif olarak incelendi. Hastaların 47'si kadın, 35'i erkek idi. Yaş ortalaması ise 65.1 olarak bulunduHastaların %74.4'ünde deri tutulumu görülürken %25.6'sında mukozal tutulum görüldü. Mukozal tutulum en sık oral mukoza tutulumu şeklinde idiÇalışmadaki 82 hastanın 5'i takipsiz olduğundan remisyon ve aktivasyon ile ilgili değerlendirmeler 77 hasta üzerinden yapıldıAralık 2007 itibariyle 77 hastanın 26'sının(%33.76) en az 1 kez aktivasyona girdiği saptandı, 27 (% 35.06) hasta ise remisyonda idiHastalar 60 yaş altı ve 60 yaş ve üstü olarak 2 gruba ayrıldığında remisyon(p=0.443) ve aktivasyon(p=0.573) görülme oranları arasındaki fark anlamlı bulunmamıştırErkek ve kadınlarda aktivasyon (p= 0.932) ve remisyon (p=0.823) görülme oranları arasında da anlamlı bir farklılık saptanmamıştırİzlem süresi 12 aydan fazla olan hastalarda izlem süresi 6-12 ay arasında olan hastalara göre remisyon görülme oranları anlamlı derecede yüksek bulundu(p=0.013Mukozal tutulum olan hastaların aktivasyona girmeleri ile mukozal tutulum olmayan hastaların aktivasyona girmeleri arasında herhangi bir farklılık saptanmaz iken(p=0.816) mukozal tutulum olmayan grupta remisyona girme oranları anlamlı derecede yüksek bulunmuştur(p=0.016Hastaların %9.8'inde malinite, %57.3'ünde ek olarak en az bir hastalık eşlik ediyordu. Ek hastalığı olan grupta aktivasyona girme oranı istatistiksel olarak anlamlı derecede yüksek bulunmuştur(p=0.028Tedavi olarak KS, KS + dapson ve KS + IS olarak 3 gruba ayrıldığında her 3 grupta aktivasyon(p=0,633) ve remisyon (p=0.157) görülme oranları arasında anlamlı bir farklılık saptanmamıştırKullanılan kortikosteroid dozları düşük(0-16mg/gün), orta (32-56 mg/gün) ve yüksek doz (60-100mg/gün) olarak 3 gruba ayrıldığında her 3 grupta remisyon görülme oranları birbirinden farklı bulunmamıştır(p=0.053Mukoza tutulumunun olmaması ve uzun izlem süresi remisyon için olumlu faktörler iken, ek bir hastalığın varlığı aktivasyonu tetikleyen faktör olarak bulunmuştur.
Bullous pemphigoid is an autoimmune bullous disease predominantly affecting people over 60 years of age. It is most commonly encountered autoimmune bullous disease in Europe. Autoantibodies directed against 180-kDa BP2 and 230-kDa BP1 are involved in the pathogenesis.A total of 82 patients with the diagnosis of bullous pemphigoid who were under follow-up between years 1987-2007 at the Bullous Diseases Outpatient Clinic of Istanbul University, Istanbul Medical Faculty, Department of Dermatology, are reviewed retrospectively in our study.Cutaneous involvement was obtained in 74.4% of the patients, whereas mucous membranes were involved only in 25.6%. Mucosal involvement was observed most commonly in the form of oral mucosa.The evaluations about the remission and activation rates were based upon the data of 77 patients because 5 from the 82 patients admitted to the study were `lost? because of incomplete follow- up.In 26 (33.76%) of the assessed 77 patients, at least one activation (p=0.573) was observed during their follow-up period until 2007 December; while 27(35.06%) were in remission at that timeWhen the patients were divided into two groups according to their age being under or over 60 years, the difference between the remission (p=0.443) and activation (p=0.573) rates was not found to be statistically significant among these different age groups.The activation (p=0.932) and remission (p=0.823) rates showed also no stasistically significant differences among women and men.In the patients with a follow-up period longer than 12 months, the remission rate was significantly higher (p=0.013) than those with a shorter follow-up period of 6-12 months.There was no difference between the activation rates among the patients with and without mucosal involvement, but the remission rate was found to be significantly high (p=0.016) in the group without mucosal involvement.Coexisted malignancy was observed in 9.8% of the patients, whereas there was at least one medical comorbidity in 57.3%. In the group with medical comorbidity, the activation rate was obtained statistically high (p=0.028).When we grouped patients according to their treatment regimens, such as corticosteroids, corticosteroids + dapson and corticosteroids + immunosuppressive groups; no statistically significant differences were observed between the activation (p=0.633) and remission (p=157) rates among these three treatment groups.If the given corticosteroid dosages were classified into 3 groups, such as the low (0-16mg/day), middle (32-56mg/day) and high (60-100mg/day) dosage groups, there were no statistically significant diffrences between the remission rates among them.The factors affecting the remission were defined as the absence of mucosal involvement and long term follow-up; whereas medical comorbidity was found to be the only factor affecting the activation.
